摘要
目的分析非酒精性脂肪性肝病(NAFLD)与幽门螺杆菌(Hp)感染的相关性。方法随机选取2016年5月-2018年1月昆明医科大学第一附属医院健康管理中心体检者3053例,收集其体检数据,并根据肝脏超声结果,将符合纳入标准者2409例分为NAFLD病组(n=741)和对照组(n=1668),比较2组Hp感染情况。应用Spearman相关分析NAFLD与Hp感染的关系。结果Hp感染总体检出率为34.21%(824/2409),NAFLD组Hp感染率较对照组明显升高(41.43%vs.31.00%,χ^2/P=24.364/<0.01)。NAFLD组BMI、ALT、AST、UA、FPG、TC、TG、LDL-C和NAFLD纤维化评分(NFS)均明显高于对照组,HDL-C低于对照组(P<0.01)。随着脂肪肝严重程度增加,Hp感染程度差异无统计学意义(P>0.05)。NAFLD组内,脂肪肝严重程度与BMI、ALT、AST、UA、TG水平呈正相关,与HDL-C水平呈负相关,差异均有统计学意义(P<0.05);脂肪肝严重程度与Hp感染程度不相关(P>0.05)。结论Hp感染与NAFLD相关,Hp感染可能是NAFLD发病的危险因素,但是NAFLD严重程度与Hp感染程度无关。
Objective To analyze the correlation between non-alcoholic fatty liver disease(NAFLD)and Helicobacter pylori(Hp)infection.Methods From May 2016 to January 2018,a total of 3053 physical examinees were randomly selected from the health management center of the First Affiliated Hospital of Kunming Medical University.According to the results of liver ultrasound,2409 patients were divided into NAFLD group(n=741)and control group(n=1668).Hp infection was compared between the two groups.The relationship between NAFLD and Hp infection was analyzed by Spearman correlation.Results The total physical examination rate of Hp infection was 34.21%(824/2409).The Hp infection rate of NAFLD group was significantly higher than that of the control group(41.43%vs.31.00%,χ^2/P=24.364/<0.01).BMI,ALT,AST,UA,FPG,TC,TG,LDL-C and NAFLD fibrosis score(NFS)in NAFLD group were significantly higher than those in the control group,while HDL-C was significantly lower than that in the control group(P<0.01).With the increase of fatty liver severity,there was no significant difference in Hp infection(P>0.05).In NAFLD group,the severity of fatty liver was positively correlated with BMI,alt,AST,UA,TG,and negatively correlated with HDL-C(P<0.05).There was no correlation between the severity of fatty liver and Hp infection(P>0.05).Conclusion Hp infection is related to NAFLD,Hp infection may be a risk factor of NAFLD,but the severity of NAFLD is not related to the degree of Hp infection.
