摘要
目的研究重症急性胰腺炎患者C-反应蛋白(CRP)及血清降钙素原(PCT)变化,为临床相关诊治提供参考。方法选取我院于2016年8月至2018年5月期间收治的90例重症急性胰腺炎作为观察对象(SAP组),另选取同期收治的90例轻症急性胰腺炎组(MAP组)和100例来院参加体检的健康体检者(对照组)。对比检测三组患者的CRP及PCT变化,并分析二者联合诊断价值。结果三组受检患者相比,SAP组患者血清CRP、PCT水平高于MAP组,且MAP组高于对照组,差异均有统计学意义(P<0.05)。血清PCT诊断的ROC曲线下降面积为0.87,最佳节点为≥3ng/mL;血清CRP诊断的ROC曲线下降面积为0.79,最佳节点为≥50mg/L。二者联合诊断的阴性似然比为0.14、阳性似然比为4.14、准确度为88.2%、特异度为86.4%、敏感度为91.5%。结论重症急性胰腺炎患者C-反应蛋白及血清降钙素原与病情发展呈正相关,且两项指标联合诊断准确度、特异度、敏感度均较高,值得临床借鉴。
Objective To study the changes of C-reactive protein(CRP)and serum calcitonin(PCT)in patients with severe acute pancreatitis(SAP),and to provide reference for clinical diagnosis and treatment.Methods 90 cases of severe acute pancreatitis in our hospital from August 2016 to May 2018 were treated as the observation objects(SAP group),90 cases of mild acute pancreatitis group were selected from(MAP group)and 100 cases of hospital to participate in the examination of healthy subjects(control group).The changes of CRP and PCT in the three groups were compared,and the combined diagnostic value of the two groups was analyzed.Results compared with the three groups,the levels of serum CRP and PCT in group SAP were higher than those in group MAP,and MAP group was higher than that in control group,the difference was statistically significant(P<0.05).The ROC curve of serum PCT in the diagnosis of the decreased area was 0.87,the best node is greater than or equal to 3ng/mL;ROC curve of serum CRP in the diagnosis of the decreased area was 0.79,the optimal node is greater than or equal to 50mg/L.The negative likelihood ratio of the two combined diagnosis was 0.14,the positive likelihood ratio was 4.14,the accuracy was 88.2%,the specificity was 86.4%,and the sensitivity was 91.5%.Conclusion the level of C-reactive protein and serum procalcitonin in patients with severe acute pancreatitis is positively correlated with the development of the disease.The accuracy,specificity and sensitivity of the combination of the two indexes are all high,which is worthy of clinical reference.
