摘要
目的通过分段读出平面回波成像(RS-EPI)序列鼻腔黏膜成像,评估基于表观扩散系数(ADC)图的影像组学在鉴别鼻腔黏膜病变中的应用价值。方法搜集41例经病理证实的鼻炎患者,另外再搜集17名鼻腔黏膜正常的志愿者作为正常对照组,根据病理把患者分为两组:慢性肥厚性鼻炎组20例与单纯性鼻炎组21例。行扩散加权成像(DWI)扫描。采用基于Matlab 2017a版本,自行编制的软件进行参数值的测量,测量的指标包括对比度、相关性、能量、一致性、均值、熵、平滑度、峰度、偏度、逆差矩(IDM)。数据行方差齐性的检验,使用SPSS 22.0软件处理,应用非参数Kruskal-Wallis检验进行组间指标的比较。应用Med Calc 13.0软件绘制受试者工作特征(ROC)曲线,P<0.05认为差异具有统计学意义,分析各个参数鉴别诊断的效能。结果对比度、均值、IDM在鉴别正常组和慢性肥厚性鼻炎组中差异具有统计学意义,对比度、偏度在鉴别单纯性鼻炎组、慢性肥厚性鼻炎组中差异具有统计学意义。ROC结果显示,对比度鉴别诊断两种不同性质的病变曲线下面积(AUC)是0.748±0.044,95%可信区间(CI)为(0.588,0.870),偏度的AUC为0.710±0.058,95%CI为(0.547,0.840)。对比度的诊断效能优于偏度,其特异度、敏感度、准确率、阴性预测值和阳性预测值分别为66.67%、80.00%、73.17%、77.78%、69.57%。结论基于RS-EPI序列ADC图影像组学在鼻腔黏膜病变诊断中,具有较大的应用价值。
Objective To investigate parameters derived from radiomics based on ADC map calculated by using readout segmentation echo-planar(RS-EP)sequence in differential diagnosis of simple rhinitis and chronic hypertrophic rhinitis.Methods A total of 41 patients confirmed by pathology were enrolled in this study and all of them underwent DWI by u-sing RS-EP sequence,which were divided into simple rhinitis group(21)and chronic hypertrophic rhinitis(20),and an-other 17 volunteers were enrolled as a control group.The contrast,correlation,energy,homogeneity,mean,entropy,smoothness,kurtosis,skewness and inverse difference moment(IDM)were calculated.Comparative statistical analysis was performed for comparing among these three groups.ROC analysis was conducted to assess the diagnostic performance.Results Significant differences were found for parameters contrast,mean,IDM in differential diagnosis of control and chronic hypertrophic rhinitis group.For parameters contrast,skewness,significant differences were also found in differenti-al diagnoses of simple rhinitis and hypertrophic rhinitis.ROC analysis demonstrated a higher area under the curve(AUC)for contrast than skewness value[(0.748±0.044)(0.588,0.870)vs(0.710±0.058)(0.547,0.840),P<0.05]with sensitivity,specificity,accuracy,positive and negative predictive values of 80.00%,66.67%,73.17%,69.57%,77.78%,respectively.Conclusion Parameters derived from radiomics based on ADC map calculated by using readout seg-mentation echo-planar(RS-EP)sequence demonstrated great value in differential diagnoses of simple rhinitis and chronic hypertrophic rhinitis.
