穴位埋线治疗原发性三叉神经痛的临床研究 被引量:7

摘要 目的探讨穴位埋线治疗原发性三叉神经痛的临床效果及机制。方法选择2015年7月~2018年3月西安医学院第二附属医院收治的60例原发性三叉神经痛的患者为研究对象,单纯进行口服西药治疗者30例设为对照组;采用穴位埋线疗法的患者30例设为穴位埋线组。比较两组的临床疗效,治疗前后的中医症状积分、视觉模拟评分(VAS)、血清中白介素-1β(IL-1β)、白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、β-内啡肽(β-EP)、血管活性肠肽(VIP)、5-羟色胺(5-HT)水平。结果穴位埋线组治疗后中医症状积分、VAS评分明显低于治疗前及对照组,治疗前后差值明显高于对照组,差异有统计学意义(P<0.01)。穴位埋线组治疗后血清IL-1β、IL-6、TNF-α、VIP水平明显低于治疗前及对照组,血清β-EP、5-HT水平明显高于治疗前及对照组,治疗前后差值均明显高于对照组,差异有统计学意义(P<0.01)。结论穴位埋线治疗原发性三叉神经痛具有肯定的治疗效果,其临床总体疗效及镇痛效果均优于口服西药治疗,其机制可能与调控原发性三叉神经痛的相关炎性因子及神经递质有关。 Objective To investigate the clinical effect and mechanism of acupoint catch embedding in the treatment of primary trigeminal neuralgia.Methods 60 patients with primary trigeminal neuralgia admitted to our hospital from July 2015 to March 2018 were included in this study.30 patients set as the control group were treated with oral western medicine alone.30 patients with acupoint catgut embedding therapy were set as acupoint catgut embedding group.Clinical efficacy,TCM symptom score,visual analogue scale(VAS),serum levels of IL-1β,IL-6,TNF-α,β-EP,VIP,5-HT were compared between the two groups before and after treatment.Results After treatment,TCM symptom scores and VAS scores in acupoint catgut embedding group were significantly lower than those before and after treatment,and the differences before and after treatment were significantly higher than those in the control group(P<0.01).After treatment,serum levels of IL-1β,IL-6,TNF-αand VIP in the acupoint embedding group were significantly lower than those in the control group and before treatment.The serum levels ofβ-EP and 5-HT were significantly higher than those in the control group,and the differences before and after treatment were significantly higher than those in the control group(P<0.01).Conclusion Acupoint catgut implantation has a positive therapeutic effect on primary trigeminal neuralgia,and its overall clinical efficacy and analgesic effect are better than that of oral western medicine treatment.The mechanism may be related to the regulation of related inflammatory factors and neurotransmitters in primary trigeminal neuralgia.
