摘要
目的:观察降糖饮对糖尿病肾病患者(气阴两虚型)的临床疗效和对尿单核细胞趋化蛋白-1(MCP-1)水平变化的影响。方法选取2017年1月~2018年7月我院内科收治的129例糖尿病肾病患者,入选患者按照随机数字法分为观察组(65例)和对照组(64例)。对照组给予基本的控制血糖,血压治疗,观察组在基础治疗的同时给予降糖饮服用,治疗3个月后观察两组患者临床症状变化,检测患者治疗前后空腹血糖(FPG)、总胆固醇(TC)、糖化血红蛋白(HbAlc)、尿MCP-1、尿白蛋白/尿肌酐(ACR)水平。结果治疗后观察组临床总有效率92.3%显著高于对照组84.4%,差异具有统计学意义(P<0.05);治疗后两组患者MCP-1、ACR、FPG、HbAlc、eGFR、Scr和中医证候积分水平均下降,且观察组低于对照,差异具有统计学意义(P<0.05);两组治疗后TC、TG、LDL值均下降,且观察组低于对照组,差异具有统计学意义(P<0.05);治疗后两组HDL均升高,且观察组高于对照组,差异具有统计学意义(P<0.05)。结论降糖饮能改善糖尿病肾病患者(气阴两虚型)临床症状,降低MCP-1表达水平,有助于改善肾功能。
Objective:To investigate the clinical effect of Jiangtang Decoction in patients with Qi-Yin deficiency type diabetic nephropathy and its effect on urinary monocyte chemoattractant protein-1(MCP-1)level change.Methods:A total of 129patients with diabetic nephropathy admitted in our hospital between January 2017and July 2018were divided into controlled group(65cases,basic blood glucose control and blood pressure treatment)and observation group(Meases,Jiangtang Decoction plus basic blood glucose control and blood pressure treatment).After 3months of treatment,the clinical symptoms of the two groups were observed.The fasting plasma gluco(FPG),total cholesterol(TC),glycosylated hemoglobin Ale(HbAlc),urine MCP-1,urinary albumin/urine creatinine(ACR)levels were measured before and after treatment.Results:After treatment,the total effective rate(92.3%)of the observation group was significantly higher than that(84.4%)of the controlled group(P<0.05).After treatment,the levels of MCP-1,ACR,FPG,HbAlc,eGFR,Sc rand TCM syndrome score were decreased in the two groups,and the observation group was lower than the controlled group(P<0.05).TC,TG and LDL values??were decreased,and the observation group was lower than the controlled group(P<0.05).The HDL of the two groups were increased after treatment,and the observation group was higher than the controlled group(P<0.05).Conclusion:Jiangtang Decoction can improve the clinical symptoms of patients with diabetic nephropathy(Qi-Yin deficiency),reduce the expression level of MCP-1,which is beneficial to improve renal function.
