超声测量6~12岁儿童呼吸末腹腔干与主动脉夹角及腹腔干血流峰值

摘要 目的通过超声测量无中弓韧带压迫综合症症状6~12岁儿童呼、吸末腹腔干与主动脉夹角及血流峰值,并评估其正常范围。方法通过超声检查测量600例(男女各300例)无MALS症状6~12岁儿童,观察腹腔干起始段走行,再分别测量腹腔干与主动脉夹角及吸气末、呼气末腹腔干收缩期峰值速度,计算呼气末与吸气末血流速度变化率。结果6~12岁儿童吸气末腹腔干与主动脉角平均为(51.8±15.6)°(95%可信区间48.4~55.2°),收缩期血流峰值速度为(97.7±14.2)cm/s(95%可信区间94.6~100.8 cm/s);呼气末腹腔干与主动脉角平均为(75.1±20.3)°(95%可信区间69.7~80.4°),收缩期血流峰值速度为(106.4±18.2)cm/s(95%可信区间102.4~110.8 cm/s)。吸气末与呼气末腹腔干血流速度变化率为(10.63±7.75)%。结论通过超声测量正常6~12岁儿童呼、吸末腹腔干与主动脉夹角,腹腔干收缩期血流峰值速度及其变化率的正常值范围有助于通过超声诊断6~12岁儿童中弓韧带压迫综合征。 Objective To measure the angle between the celiac trunk and aorta,and measure the peak value of blood flow of celiac trunk in children aged 6~12 years old,which without median arcuate ligament syndrome(MALS)symptoms,evaluated its normal range.Methods 600 children(300 males and 300 females)aged 6~12 years old without MALS symptoms were examined by ultrasonography.The initial segment of the celiac trunk was observed.The angle between the celiac trunk and aorta and the peak systolic velocity of maximum end-inspiration and end-expiration was measured,and the velocity variation rate between maximum end-inspiration and end-expiration was calculated.Results The mean value of the angle between celiac trunk and aortic angle of maximum end-inspiration was(51.8±15.6)°(95%confidence interval 48.4~55.2°)and the peak systolic blood flow velocity was(97.7±14.2)cm/s(94.6~100.8 cm/s).The mean value of the angle between celiac trunk and aortic angle of maximum end-expiration was(75.1±20.3)°(95%confidence interval 69.7~80.4°)and the peak systolic blood flow velocity was(106.4±18.2)cm/s(95%confidence interval 102.4~110.8 cm/s).The velocity variation rate between maximum end-inspiration and end-expiration was(10.63±7.75)%.Conclusion Measuring the angle between the abdominal trunk and aorta,the peak velocity of blood flow in the abdominal trunk during systole and the normal range of its change rate by ultrasound in normal children aged 6~12 years are helpful to diagnose the syndrome of middle arch ligament compression in children aged 6~12 years old by ultrasound.
