摘要
目的探讨肌腱"Z"形延长术治疗小儿晚期轻中型前臂缺血性肌挛缩致手指畸形的临床效果。方法 2010年1月-2015年12月,对12例晚期轻中型前臂缺血性肌挛缩致手指畸形患者采用肌腱"Z"形切断,延长后行断端直接缝合术,术后按肌腱修复术后制动及功能训练。结果术后伤口愈合好,无切口并发症发生。12例获随访10~36个月,平均24个月。随访患者术后患指及腕关节主被动伸直受限、屈曲畸形明显改善,屈伸肌力5级。按照中华医学会手外科学会上肢部分功能评定试用标准进行疗效评估:优10例,良2例,优良率100%。结论应用肌腱"Z"形延长术治疗小儿晚期轻中型前臂缺血性肌挛缩致手指畸形是一种简单、实用、理想的手术方式。
Objective To explore the clinical efficacy of Z shaped lengthening of tendon on terminal mild or medium Ischemic contracture deformity of forearm in children. Methods From January 2010 to December 2015, operation of Z shaped lengthening of tendon was employed for 12 cases of terminal mild or medium Ischemic contracture deformity of forearm in children. Results The wound healed well and no incision complications occurred after operation. 12 patients were followed up from 10 to 36 months,with an average of 24 months. Patients had improvement in the restriction of active and passive flexion and extension. Flexion and extension strength was 5 degree. According to the evaluation criteria of upper limb function of the Chinese Medical Association of hand surgery: Excellent in 10 cases, good in 2 cases,excellent and good rate was 100%. Conclusion The operation of Z shaped lengthening of tendon can effectively repair terminal mild or medium Ischemic contracture deformity of forearm in children, and is a simple and practical method.
