摘要
中国家庭医生签约服务制度起步晚,但发展迅速,已有较为成熟的“1+1+1”模式、“三师共管”模式、“四零”模式和“医养护一体化”模式。与相对起步较早有着深厚经验的国家相比,我国目前仍然存在签约覆盖率不高、医保支付方式不完善、绩效考核制度不完备等问题。文章旨在通过对目前国内典型制度及模式的概述来归纳出当前我国家庭医生签约制度所存在的不足并提出相应对策,从而提高基本医疗服务、强化基层卫生水平、建立健全医疗改革机制。
Although Chinese family doctor signing service system started late,it developed rapidly and fast,appearing a more mature“1+1+1”model,“three division coordinated”model,“Four-Disappearance”model and“Medical treatment integrated with maintenance”model.Compared with experienced countries which began earlier,there exists some issues in the current system,such as low contract coverage,imperfect payment of medical insurance and incomplete performance appraisal system.This paper is to sum up the shortcomings of the current family doctor signing system in China through the typical domestic system and model,and offer some corresponding suggestions in order to improve the basic medical service,strengthen the fundamental health care,and establish the mechanism of medical reform.
