摘要
目的分析高龄妇女抗苗勒管激素(Anti-Mullerian hormone,AMH)预测助孕治疗的结局。方法选取本院门诊于2017年5月至2018年12月接收的≥35岁妊娠高龄女性168例。其中将AMH值正常的98例自然受孕女性设为观察组,70例AMH<1.0ng/mL助孕女性为对照组。对比两组自然受孕率、促排卵治疗率、流产率,同时分析不同年龄段患者的AMH值以及AMH<1.1ng/mL的比例。结果观察组自然受孕率43.88%、试管婴儿成功率50.00%高于对照组的27.14%和31.43%,促排卵治疗率56.12%、流产率55.10%低于对照组的92.85%和85.71%(P<0.05);41~45岁患者AMH水平(3.10±1.42)ng/mL低于35~40岁(5.22±2.24)ng/mL,同时41~45年龄段人群的AMH<1.1ng/mL比例为58.11%高于35~40岁的14.89%(P<0.05)。结论卵巢储备功能越佳,AMH越高,同时自然受孕率也有所提升。因此,高龄妇女通过AMH检测对卵巢储备功能测定,有利于对高龄备孕妇女受孕机率进行评定,降低妊娠风险性。同时AMH低下的妇女行试管婴儿的成功率有所提升。
Objective To analyze the predictive effect of anti-Mullerian hormone(AMH)on the outcome of assisted pregnancy in elderly wom en.Methods 168 elderly women aged≥35 years old who were admitted to our hospital from May 2017 to December 2018.Among them,98 natural pregnant women with normal AMH values were set as observation group,and 70 AMH<1.0 ng/mL pregnant women were used as control group.The natural conception rate,ovulation induction rate,and abortion rate were compared between the two groups.The AMH values of patients with different age groups and the ratio of AMH<1.1 ng/mL were compared.Results The observation group natural pregnancy rate was 43.88%and the success rate of test-tube infants was 50.00%,higher than the control group 27.14%and 31.43%,the ovulation induction rate was 56.12%and the abortion rate was 55.10%,lower than the control group 92.85%and 85.71%(P<0.05).The AMH level of patients aged 41-45 years(3.10±1.42)ng/mL was lower than that of 35-40 years(5.22±2.24)ng/mL,and the proportion of AMH<1.1 ng/mL in the 41-45 years group 58.11%.The ratio was higher than 35-40 years 14.89%,and the comparison between groups was statistically significant(P<0.05).Conclusion The better the ovarian reserve function,the higher the AMH and the higher the natural pregnancy rate.Therefore,the determination of ovarian reserve function by AMH test is helpful to evalu ate the pregnancy probability of the elderly pregnant women and reduce the risk of pregnancy.At the same time,the success rate of IVF for women with low AMH has increased.At the same time,the success rate of test tube babies in women with low AMH has improved.
