磁共振成像在内耳畸形构成及术前评估中的应用 被引量:8

The magnetic resonance study of the composition and preoperative evaluation of inner ear malformation

摘要 目的探讨磁共振成像(MRI)在内耳畸形诊断及分型中的应用价值,为感音神经性聋(sensorineural hearing loss,SNHL)患者提供人工耳蜗植入术(CI)前的可靠影像学资料。方法回顾性分析2014年8月~2017年12月间于我院就诊的314例(628耳)SNHL患者MRI水成像图像资料,采用Sennaroglu分类方法对内耳畸形患者进行诊断及分类,并分析SNHL患者的治疗情况。结果MRI对内耳畸形的检出率较高,内耳畸形中单纯前庭导水管畸形比率最高,半规管畸形比率次之,耳蜗畸形比率居第三位,内耳畸形患者的性别构成没有差异(P>0.05)。行CI的患者占所有就诊人数的47.77%。结论MRI作为一种无创检查技术可为患者内耳有无畸形及畸形的类型提供可靠的诊断信息,并为患者人工耳蜗植入术提供重要的影像资料。 Objective To investigate the values of MR fluid imaging in the classification and diagnosis of inner ear malformations, in order to provide accurate preoperative imaging data for sensorineural hearing loss(SNHL) patients’ cochlear implants. Methods Retrospective analysis of MR fluid imaging data about 314 patients(628 ears) with severe sensorineural hearing loss who admitted our hospital from August 2014 to December 2017 were made. The Sennaroglu classification criteria was used to diagnose and classify the inner ear malformations, and the treatment data of SNHL patients were collected and analyzed. Results The detection rate of inner ear malformation was higher in MRI, the ratio of simple vestibular aqueduct deformity was the highest, the semicircular canal deformity was the second, and the cochlear deformity was the third. There was no difference in sex composition between the patients with inner ear malformation(P>0.05). Cochlear implant(CI) patients accounted for 47.77% of all visits. Conclusion As a noninvasive technique, MRI can provide reliable diagnostic information on the types of deformities for the patients and provide essential imaging data for the CI.
