摘要
目的:探究膝关节镜手术中选择经超声引导下股神经联合侧入路腘窝坐骨神经阻滞的临床效果。方法:2018年1-12月收治膝关节镜手术患者90例,随机分为三组各30例。A组给予超声引导股神经-坐骨神经阻滞;B组给予超声引导腰丛-坐骨神经阻滞;C组给予连续硬膜外麻醉处理。比较三组麻醉效果。结果:B组Ta较A、C组明显更长,C组Tb、Td较A、B组更短,C组Te、Tg较A、B组更长,差异均有统计学意义(P<0.05);C组T1~T3平均动脉压(MAP)水平显著低于A、B组,差异有统计学意义(P<0.05);C组麻醉后不良反应率显著高于A、B组,差异有统计学意义(P<0.05)。结论:膝关节镜手术中选择超声引导股神经联合侧入路腘窝坐骨神经阻滞效果明确,且安全性相对更高。
Objective:To explore the clinical effect of ultrasound-guided femoral nerve combined lateral approach popliteal sciatic nerve block in knee arthroscopy.Methods:From January to December 2018,90 cases of knee arthroscopy were enrolled,they were randomly divided into three groups,30 cases in each.Group A was given an ultrasound-guided femoral nerve-sciatic nerve block;Group B was given ultrasound-guided lumbar plexus-sciatic nerve block;Group C was treated with continuous epidural anesthesia.The anesthetic effects among the three groups were compared.Results:Ta in group B was significantly longer than in group A and group C,Tb,Td in group C was shorter than in group A and group B,Te,Tg in group C was longer than in group A and group B,the differences were statistically significant(P<0.05);The level of T1~T3 MAP in group C was significantly lower than in group A and group B,the difference was statistically significant(P<0.05);The adverse reaction rate after anesthesia in group C was significantly higher than in group A and group B,the difference was statistically significant(P<0.05).Conclusion:Ultrasound-guided femoral nerve combined lateral approach popliteal sciatic nerve block for knee arthroscopy has a clear effect and it is relatively safer.
