摘要
目的:探讨影响心血液透析患者心血管事件预后的相关因素。方法:2015年5月选取长期维持性血液透析患者105例,随访至2018年5月,统计患者1、2、3年生存率并计算中位生存期(MST),对患者信息进行单因素分析。结果:105例维持性血液透析患者中位生存期为36个月,其中无抑郁、轻度抑郁、中度抑郁、重度抑郁患者MST分别为36.00、36.00、14.00、19.00个月。1、2、3年生存率分别为97.14%、87.62%、74.39%。单因素分析显示,患者年龄、贫血程度、肺部感染、呼吸衰竭、抑郁、焦虑、白蛋白水平、低钠血症为心血管事件预后相关因素。多因素分析显示,抑郁是影响血液透析患者预后的独立危险因素。结论:抑郁是血液透析患者生存期的重要影响因素,中度抑郁生存期较轻度、无抑郁患者明显缩短。
Objective:To explore the related factors of influence on the prognosis of cardiovascular events in hemodialysis patients.Methods:In May 2015,105 patients with long-term maintenance hemodialysis were selected and followed up to may 2018,we counted the 1,2,and 3 years survival rates of patients,calculated the median survival time MST,and analyzed the information of patients by single factor analysis.Results:The median survival time of 105 patients with maintenance hemodialysis was 36 months,the MST of patients without depression,mild depression,moderate depression and severe depression were 36.00,36.00,14.00 and 19.00 months respectively.The 1,2,and 3 years survival rates were 97.14%,87.62%,and 74.39%respectively.Univariate analysis showed that age,degree of anemia,pulmonary infection,respiratory failure,depression,anxiety,albumin level and hyponatremia were the prognostic factors of cardiovascular events.Multivariate analysis showed that depression was an independent risk factor for the prognosis of hemodialysis patients.Conclusion:Depression is an important factor in the survival of hemodialysis patients,compared with mild depression and non depression patients,the survival time of moderate depression patients is significantly shorter.
