摘要
目的探讨老年慢性肾脏病(chronic kidney diseas,CKD)患者外周血中性粒细胞/淋巴细胞比值(neutrophil to lymphocyte ratio,NLR)与心脏瓣膜钙化(cardiac valve calcification,CVC)的相关性。方法测定134例非透析老年CKD患者超敏C反应蛋白(high-sensitivity C-reactive protein,hsCRP)、血常规及其他相关指标,并计算NLR值。超声心动图检测是否伴有CVC,并进行相关性分析。结果与非钙化组相比,钙化组患者年龄、NLR、hsCRP水平较高,有统计学差异,且NLR与年龄、hs CRP、白细胞计数、血清尿素氮、肌酐呈正相关,与血红蛋白、血清白蛋白、总胆固醇、三酰甘油呈负相关。Logistic回归分析显示,NLR水平升高(OR=2.182,95%1.109~4.290,P<0.05)是老年CKD患者CVC的独立危险因素。结论老年CKD患者NLR与hsCRP及年龄、白细胞计数、血清尿素氮、肌酐呈正相关,NLR在CVC患者中显著高于非CVC组,且NLR水平升高是老年CKD患者CVC的独立危险因素。
Objective To explore the correlation of neutrophil to lymphocyte ratio(NLR)in peripheral blood with high-sensitivity C-reactive protein(hsCRP)and cardiac valve calcification(CVC)in the elderly with chronic kidney disease(CKD).Methods The level of HsCRP,complete blood count and other index of 134 elderly with CKD(non-dialysis)were detected and the NLR values were calculated;echocardiogram was applied in checking the occurrence of cardiac valve calcification and the correlative analysis was performed.Results Age,NLR and hsCRP levels of the elderly with calcification were significantly higher than those of the elderly with no calcification;NLR was positively correlated with age,hsCRP,WBC,BUN,Scr,and negatively correlated with Hb,Alb,TC and TG;multivariate logistic regression analysis showed that elevated NLR level(OR=2.182,95%:1.109-4.290,P<0.05)was an independent risk factor of cardiac valve calcification in the elderly with CKD.Conclusions The NLR of the elderly with CKD is in positive correlation with hsCRP,age,white blood cell count,serum urea nitrogen and creatinine;the NLR in CVC cases is significantly higher than that in non-CVC cases and the higher NLR is the independent risk factor of CVC in the elderly with CKD.
