摘要
目的探讨多层次护理管理模式在静脉治疗专科护理门诊中的应用效果。方法将于我院静脉治疗门诊留置中心静脉导管的2661例次患者按就诊时间先后分为对照组及观察组,对照组1237例次采取常规管理模式,观察组1424例次采取多层次护理管理模式,比较两组患者维护就医体验情况及患者导管并发症发生情况。结果对照组和观察组患者导管并发症发生情况:穿刺处皮疹分别为261(9.81%)及90(3.38%)、贴膜固定不规范分别为230(8.64%)及74(2.78%)、穿刺部位感染分别为73(2.74%)及21(0.78%)、医用粘胶相关皮损分别为71(2.67%)及32(2.74%),各项目分别比较差异均有统计学意义(P<0.05)。对照组和观察组患者就医维护体验评价得分情况:就诊环境分别为(7.32±0.31)及(9.04±0.23)、就诊秩序分别为(7.74±0.42)及(9.24±0.40)、服务流程分别为(7.53±0.13)及(9.61±0.20)、服务质量分别为(7.14±0.74)及(9.36±0.10),各项目分别比较,差异均有统计学意义(P<0.05)。结论多层次护理管理模式应用于静脉治疗专科门诊,能减少患者并发症的发生,提高就医体验的满意程度。
Objective To explore the effects of application of a high quality multilevel nursing management model in the intravenous therapy specialist nursing clinic.Methods A total of 2661 patients who were given indwelling central venous catheters in intravenous therapy clinic of our hospital were divided into control group and observation group according to the visiting order.Control group(1237 case-time)was given routine management model,and observation group(1424 case-time)was given high quality multilevel nursing management model.The medical maintenance experience and occurrence of catheter complications of patients were compared between the two groups.Results After nursing,the situation of catheter complication rate in control and observation group were as follows.The quantity of patients with puncture rash in those two groups were 261(9.81%)and 90(3.38%)respectively,with Nonstandard adhesive fixation were 230(8.64%)and 74(2.78%)respectively,with puncture site infection were 73(2.74%)and 21(0.78%)respectively,with Skin lesion because of Medical viscose were 71(2.67%)and 32(2.74%)respectively.The differences of each item were statistically significant(P<0.05).After nursing,the scores of medical maintenance experience of medical environment,treatment order,service procedure and service quality in control group and observation were(7.32±0.31)points and(9.04±0.23)points,(7.74±0.42)points and(9.24±0.40)points,(7.53±0.13)points and(9.61±0.20)points,and(7.14±0.74)points and(9.36±0.10)points(P<0.05).After nursing,the number of cases with catheter complications of rash at puncture site,irregular film fixation,infection at puncture site and mucilage-related skin lesions in observation group were significantly lower than those in control group(P<0.05).Conclusion High quality multilevel nursing management model in intravenous therapy specialist clinic can reduce the occurrence of complications and improve the satisfaction of medical experience.
