摘要
目的:分析医药分开综合改革前后对肿瘤患者住院费用及费用结构的影响,调整费用结构,寻找医院管理重点,以管理促发展。方法:收集北京市某肿瘤专科医院4个临床路径病种的住院费用数据,采用t检验和中断时间序列分析方法对改革前后住院费用数据进行分析,比较改革前后费用的差异情况。结果:住院总费用变化差异无统计学意义,各病种药品费用明显下降且差异有统计学意义、耗材费用均明显增加且各病种结果差异均有统计学意义。结论:医药分开综合改革的实施,使得住院总费用基本不变的情况下,合理地调整医疗费用结构、降低了药品费用、提升了总体的医疗服务能力;耗材费用的增长提示耗材费用的控制和管理成为下一步的管理重点;医院内部管理质量得到提升的同时也需要寻找新的经济增长途径,实现医院的可持续发展。
Objective:To analyze the impact of the comprehensive reform of the treatment and medicine separation on the hospitalization cost and cost structure of cancer patients,adjust the cost structure,find the focus of hospital management,and promote development by management.Methods:The hospitalization cost data of 4 clinical pathway diseases in a cancer specialist hospital in Beijing were collected,and the t-test and interruption time series analysis methods were used to analyze the hospitalization cost data before and after the reform to compare the difference in costs before and after the reform.Results:There was no statistically significant change in the total cost of hospitalization.The drug costs of each disease group decreased significantly,and the differences were statistically significant.The cost of consumables increased significantly,and the results of each disease group were statistically significant.Conclusion:The implementation of the comprehensive reform of the separation of medicines has made the total cost of hospitalization basically unchanged,rationally adjusted the structure of medical expenses,reduced the cost of medicines,and improved the overall medical service capacity;the increase in the cost of consumables has prompted the control of Management has become the next management focus;while the quality of hospital internal management has been improved,it is also necessary to find new economic growth paths to achieve sustainable development of the hospital.
