摘要
目的分析高龄经产妇孕期健康促进行为与家庭功能、心理一致感的关系,深入探讨健康促进行为的影响因素。方法选取2018年1月至6月在山东省某三级甲等医院产科门诊进行产前检查的225名高龄经产孕妇作为调查对象,采用一般资料调查问卷、健康促进生活方式量表II、家庭亲密度和适应性量表中文版以及心理一致感问卷进行研究。结果高龄经产妇孕期健康促进行为均分为(2.65±0.38)分,处于良好偏下水平。Pearson相关分析显示,家庭功能与健康促进行为呈中度正相关(r=0.564,P<0.01),心理一致感与健康促进行为呈低度正相关(r=0.326,P<0.01)。结构方程模型显示,家庭功能影响健康促进行为的过程中,标准化总效应为0.629,标准化直接效应为0.564,标准化间接效应为0.065,其中中介效应占比为10.33%,心理一致感在家庭功能对健康促进行为的影响过程中发挥部分中介效应。结论高龄经产妇孕期健康促进行为处于良好偏下水平,需要进一步提高;高龄经产妇孕期家庭功能越好,心理一致感越高,健康促进行为水平越好;心理一致感在家庭功能对健康促进行为的影响过程中发挥部分中介作用。
Objective To investigate the relationship among health promotion behavior,family function and the sense of coherence during elderly multiparas’ pregnancy,and to explore the influencing factors of health promotion behavior.Methods A total of 225 elderly multiparas who were eligible for inclusion in the obstetrics clinic of a 3 A-level hospital in Shandong Province from January 2018 to June 2018 were selected as subjects.They were surveyed using a general data questionnaire,Health Promoting Lifestyle Profile II,Family Adapt-ability and Cohesion Evaluation Scales II-Chinese Version,and Sense of Coherence-13.Results The total score of health promotion behaviors for elderly multiparas during pregnancy was(2.65±0.38).Pearson correlation analysis showed that there was a moderate positive correlation between family function and health promotion behavior(r=0.564,P<0.01),and there was a low positive correlation between sense of coherence and health promotion behavior(r=0.326,P<0.01).Structural equation model showed that the total effect of family function on health promotion behavior was 0.629,the standardized direct effect 0.564,the standardized indirect effect 0.065,and the mediating effect 10.33%.Sense of coherence played a partial mediating role during the process.Conclusion Health promotion of elderly multiparas during pregnancy is in a good sub-level,yet still needs further improvement.The better the maternal family function,the higher the sense of coherence and the better the level of health promotion during pregnancy.Sense of coherence plays a partial mediating role in the influencing process of family function on health promotion behavior during pregnancy.
