Predictive Value of Serum Progesterone-induced Blocking Factor for Preterm Delivery

摘要 通过分析母体血清中孕酮诱导阻断因子(PIBF)的浓度,对采样和早产之间的时间间隔进行预测。对37名有先兆早产孕妇和41名健康孕妇在孕第24~28周期间进行了一项前瞻性的研究工作。在37名患有征兆子痫的患者中有11名患者早产。研究中将所有孕妇分为3组:早产孕妇、先兆早产孕妇和健康孕妇。在分娩前5天内采集的样本的PIBF浓度(22.17±6.19)ng/mL明显低于之前采集的样本浓度(31.33±6.78)ng/mL,并且明显低于健康孕妇采集的样本浓度(69.65±12.38)ng/mL;采样和分娩之间的平均时间间隔为(4.1±1.8)d。多元回归分析每个观测值对早产起到的作用,结果表明PIBF的缺乏对早产有显著影响(p=0.002)。采用受试者工作特征(ROC)分析来评估PIBF的诊断准确性。ROC曲线的面积(AUC)为0.956(95%CI=0.884~0.989;p<0.0001),因此PIBF在预测早产上具有优异的诊断价值。在分娩前5天内,可以通过低于正常妊娠的PIBF值来预测妊娠终止,因而有助于有助于早产的诊断。 The time interval between sampling and preterm birth was predicted by analyzing the concentration of progesterone-induced blocking factor(PIBF)in maternal serum.A prospective work was performed on 37 pregnant women with threatened preterm labor and 41 healthy pregnant women during the 24 th to 28 th week of pregnancy.Eleven of the 37 patients with preeclampsia were premature.In the study,all pregnant women were divided into three groups:preterm pregnant women,threatened preterm pregnant women,and healthy pregnant women.The concentration of PIBF(22.17±6.19)ng/mL in samples collected within 5 days before delivery was significantly lower than that in samples collected before(31.33±6.78)ng/mL,and significantly lower than that in healthy pregnant women(69.65±12.38)ng/mL;The average time between sampling and delivery was(4.1±1.8)d Multiple regression analysis of the effect of each observation on preterm birth showed that the absence of PIBF had a significant effect on preterm birth(p=0.002).The receiver operating characteristic(ROC)analysis was used to assess the diagnostic accuracy of PIBF.The area of the ROC curve(AUC)was 0.956(95%CI=0.884~0.989;p<0.0001),so the PIBF has excellent diagnostic value in predicting preterm birth.Preterm termination can be predicted by the PIBF value below the normal pregnancy within 5 days before delivery,thus helping to diagnose preterm delivery.
