摘要
目的:探究子宫内膜癌初次手术后化疗对阴道微生态的影响并分析其与阴道残端感染的关系。方法:回顾性分析2010年1月-2014年3月本院予以术后化疗(观察组)及未予以术后化疗(对照组)的子宫内膜癌手术患者各64例的临床资料。记录两组术后6个月阴道微生态情况和阴道残端感染状况差异,比较两组5年总生存率及无瘤生存率差异,并使用Kaplan-Meier法绘制生存曲线,评估两组术后生存情况。结果:术后6个月时,观察组阴道清洁度异常率、病原体感染率及阴道残端感染发生率均高于对照组(P<0.05)。术后5年时,观察组总生存率与无瘤生存率均高于对照组(P<0.05)。结论:子宫内膜癌初次手术后化疗可破坏阴道微生态平衡,并增加阴道残端感染风险,但化疗能延长患者生存时间,于患者远期预后有利。
Objective: To explore the effect of chemotherapy on vaginal microecology after initial surgery for endometrial cancer and to analyze its relationship with vaginal stump infection. Method: The clinical data of 64 patients with endometrial cancer who underwent postoperative chemotherapy(the observation group) and no postoperative chemotherapy(the control group) from January 2010 to March 2014 were retrospectively analyzed. The differences of vaginal microecosystem and vaginal stump infection were recorded in two groups at 6 months after operation. The 5-year overall survival rate and tumor-free survival rate were compared between two groups. The survival curve was drawn by Kaplan-Meier method. Postoperative survival was evaluated in two groups. Result: At 6 months after operation, the abnormal rate of vaginal cleanliness, pathogen infection rate and vaginal stump infection in the observation group were higher than those in control group(P<0.05). At 5 years after operation, the overall survival rate and tumor-free survival rate in the observation group were higher than those in control group(P<0.05). Conclusion: Chemotherapy after initial surgery for endometrial cancer can destroy the vaginal microecological balance and increase the risk of vaginal stump infection, but chemotherapy can prolong the survival time and benefit the long-term prognosis of patients.
