摘要
目的探讨彩色多普勒超声联合血浆D-二聚体(D-D)诊断下肢静脉血栓的临床价值。方法选择2018年1月至2019年1月间韩城市人民医院诊治的92例疑似下肢静脉血栓患者,所有患者均行下肢静脉彩色多普勒超声检查和血清D-D水平检测,以下肢静脉造影结果为诊断标准,分析患者的彩色多普勒超声表现及血浆D-D水平,并采用受试者工作特征曲线(ROC)分析观察两种检查方法对下肢静脉血栓的诊断效力。结果92例疑似下肢静脉血栓患者经下肢静脉造影检出下肢静脉血栓61例,其中Ⅰ级29例,占47.54%,Ⅱ级20例,占32.79%,Ⅲ级12例,占19.67%;存在下肢静脉血栓组患者CFV、SFV、PFV、GSV内径、肌层增厚度、皮下组织增厚度、血浆D-D水平均大于无血栓组,差异有统计学意义(P<0.05),且CFV、SFV、GSV内径、皮下组织增厚度、血浆D-D水平均随静脉血栓分级的增高而增加,差异有统计学意义(P<0.05)。;彩色多普勒超声诊断下肢静脉血栓48例,灵敏度为78.68%,特异度为77.42%,曲线下面积(AUC)为0.781,血浆D-D诊断下肢静脉血栓37例,灵敏度为60.65%,特异度为70.97%,AUC为0.658,联合彩色多普勒超声检查和血浆D-D诊断下肢静脉血栓57例,灵敏度为93.44%,特异度为96.77%,AUC为0.951。结论彩色多普勒超声检查联合血浆D-D可提高下肢静脉血栓检出率,优于单独诊断。
Objective To investigate the clinical value of color Doppler ultrasound combined with plasma D-dimer(D-D)in the diagnosis of the lower extremity venous thrombosis.Methods From January 2018 to January 2019,92 patients with suspected lower extremity venous thrombosis,who admitted to Hancheng People's Hospital,were selected as the research subjects.All patients were examined by color Doppler ultrasound and serum D-D level.Taking the results of venography of the lower extremity as the diagnostic criteria,the results of Color Doppler ultrasound and plasma D-D level were estimated,and ROC was used to analyze the diagnostic effect of the two methods for the lower extremity venous thrombosis.Results Among the 92 suspected patients,61 cases were detected by venography,including 29 cases of gradeⅠ(47.54%),20 cases of gradeⅡ(32.79%),12 cases of gradeⅢ(19.67%);the mean diameter of common femoral vein(CFV),superficial femoral vein(SFV),profunda femoris vein(PFV),greater saphenous vein(GSV),myometrial thickening,subcutaneous tissue thickening and plasma D-D water in the patients with lower extremity venous thrombosis were significantly higher than those in the patients without thrombosis(all P<0.05);the diameter of CFV,SFV and GSV,the thickness of subcutaneous tissue,and the level of D-D in plasma all increased with the increase of venous thrombus grade,and the differences were statistically significant(all P<0.05).There were 48 cases of lower extremity venous thrombosis diagnosed by color Doppler ultrasound with the sensitivity of 78.68%,the specificity of 77.42%,and AUC of 0.781;there were 37 cases of lower extremity venous thrombosis diagnosed by D-D with the sensitivity and specificity of 60.65%and 70.97%,respectively,and AUC of 0.658;there were 57 cases of lower extremity venous thrombosis diagnosed by the combination of the two methods,with the sensitivity of 93.44%,the specificity of 96.77%,and AUC of 0.951.Conclusion Color Doppler ultrasound combined with plasma D-D can improve the detection rate of the lower ext
