两种不同内固定方法治疗粉碎性髌骨骨折疗效分析 被引量:2

摘要 目的分析并比较空心螺钉张力带、镍钛弹力聚髌器结合空心拉力螺钉两种内固定方法治疗粉碎性髌骨骨折的临床疗效。方法回顾性分析2013年10月~2018年10月我院收治的74例粉碎性髌骨骨折患者的临床资料,按照内固定方法不同,将其分为对照组和观察组,并比较两组患者的一般资料、手术时间、随访时间、住院费用、术后并发症发生情况、骨折愈合时间及末次随访时膝关节功能等资料,并进行统计学分析。膝关节功能采用美国特种外科医院(United States Special Surgical Hospital,HSS) 膝关节百分评分系统进行疗效评价。结果两组患者均获有效随访,两组手术持续时间、术中失血量差异无统计学意义(P>0.05);观察组骨折平均愈合时间短于对照组,差异有统计学意义(P<0.05);观察组术后并发症明显少于对照组,末次随访膝关节功能评价及住院费用镍钛弹力聚髌器结合空心拉力螺钉组高于空心螺钉张力带组,差异具有统计学意义(P<0.05)。结论镍钛弹力聚髌器结合空心拉力螺钉治疗粉碎性髌骨骨折临床疗效显著、手术操作简单、并发症少,可早期康复功能锻炼,膝关节功能恢复优良率高,具有较大的临床应用价值。 Objective To analyze and compare the clinical effect of hollow screw tension belt and nickel titanium elastic patella combined with hollow tension screw. Methods The clinical data of 74 patients with comminuted patella fracture from Oct 2013 to Oct 2018 were analyzed retrospectively, according to the internal fixation method, they were divided into control group and observation group. The data of general data, operation time, follow-up time, hospitalization cost, postoperative complications, fracture healing time and knee function at the last follow-up were compared and statistically analyzed. The knee joint function was evaluated by the knee joint score system of the United States Special Surgical Hospital (HSS). Results There was no significant difference in duration of operation and loss of blood between the two groups (P>0.05). The mean time of fracture healing in observation group was shorter than control group, the postoperative complication in observation group was less than control group (P<0.05). Evaluation of knee function and hospitalization cost of observation group was higher than control group, the difference was statistically significant (P<0.05). Conclusion The treatment of comminuted patella fracture with nickel titanium elastic patella and hollow tension screw internal fixation has good clinical effect, simple operation, few complications, early rehabilitation exercise, high recovery rate of knee function and great clinical application value.
