摘要
目的:探讨子宫内膜癌的ADC值与肌层浸润深度的相关性。方法:回顾性分析40例子宫内膜癌的术前MRI及DWI资料,观察肿瘤常规MRI表现、肌层浸润深度,测量其ADC值,并与病理所示肌层浸润深度对照分析。结果:40例子宫内膜不同程度增厚,T1WI呈等信号,T2WI呈稍高信号,DWI呈高信号。22例病灶局限于内膜或浅肌层侵犯,18例病灶侵犯深肌层,DWI联合常规MRI诊断肌层浸润深度的准确率显著高于常规MRI(P<0.05)。深肌层浸润组ADC值低于无或浅肌层浸润组(P<0.05)。结论:DWI的ADC值与肌层浸润深度具有一定相关性,有助于提高子宫内膜癌浸润深度评价的准确性,可为术前分期和手术方案制订及治疗效果评价提供依据。
Objective:To investigate the correlation of the ADC value with the invasion depth of muscle layer for endometrial carcinoma. Methods:Preoperative MRI and DWI data of 40 cases of intrauterine carcinoma were retrospectively analyzed. MRI images of tumor and depth of muscle layer invasion were observed,ADC values of different depths of muscle layer invasion were measured,and compared with pathological results. Results:A total of 18 cases of lesions invaded the deep muscle layer,the endometrium was significantly thickened,T1 WI showed equal signal,T2 WI showed high signal,and DWI showed significantly high signal after diffusion. In 22 cases,the lesions were confined to the invasion of intima or superficial muscle layer,the endometrium was slightly thickened,T1 WI showed equal signal,T2 WI showed high signal,and the diffusion degree was not as high as the muscular layer after diffusion. The accuracy of DWI combined with MRI in diagnosing the depth of muscle layer infiltration was significantly higher than the accuracy of MRI in diagnosing the depth of muscle layer infiltration(P<0.05). ADC values in the deep muscle layer infiltration group were lower than those in the no or superficial muscle layer infiltration group(P<0.05). Conclusions:The ADC value of DWI sequence has a certain correlation with the infiltrating muscle layer depth,which is helpful to improve the accuracy of the evaluation of the infiltrating depth of endometrial cancer,and provide a basis for preoperative staging,surgical planning and therapeutic effect evaluation.
