2012~2016年江苏丹阳市省级土源性线虫病监测点监测结果分析

摘要 目的分析丹阳市省级土源性线虫病监测数据,掌握流行动态和规律,为科学制订防控措施提供依据。方法用随机抽样法选择界牌镇界北村作为江苏省土源性线虫病监测点,以3周岁以上常住居民为监测对象,应用改良加藤厚涂片法和透明胶纸肛拭法检测寄生虫感染情况。结果 2012~2016年,采用改良加藤厚涂片法监测4 972人,检出土源性线虫感染者30人,感染虫种主要为蛔虫和鞭虫,均为单虫感染。感染者主要为轻度和中度感染,其中蛔虫EPG范围为64~26 268,鞭虫EPG范围为32~256。人群土源性线虫总感染率、蛔虫感染率和鞭虫感染率均呈下降趋势,分别由2012年的1.07%、0.68%和0.39%下降至2016年的0.29%、0.29%和0。人群土源性线虫总感染率、人群蛔虫感染率以及外地人口土源性线虫总感染率均呈线性下降趋势(χ^2=7.585、4.162、5.855,P<0.05)。采用透明胶纸肛拭法监测3~12周岁儿童2 323人,检出蛲虫感染11例,且均为外地儿童。儿童蛲虫感染率呈下降趋势,由2012年的1.05%下降至2016年的0。结论界北村土源性线虫感染率较低,应加强对外来流动人群的疾病监测和健教宣传。 Objective To analyze the monitoring data of soil-borne nematodiasis in Danyang city, and understand the epidemic trends and patterns of this infection for evidences to develop scientific control measures. Methods By random sampling, we included Jiebei village under the administration of Jiepai town as the monitoring site for soil-transmitted nematodiasis in Jiangsu province.Permanent residents over 3 years old were recruitedas monitoring subjects to undergo detection of the nematode eggs using modified Kato-Katz thick smear and anal swab with adhesive transparent film. Results Totally, 4 972 subjects received tests of modified Kato-Katz thick smear between 2012 and 2016, in whom 30 were found to be infected with soil-transmitted. nematodes, primarily including Ascaris lumbricoides and Trichinella trichiura. The infection was generally mild and moderate, and the eggs per gram(EPG) ranged from 64 to 26 268 for Ascaris and from 32 to 256 for Trichuris. The total infection rate of soil-borne nematodes, Ascaris and Trichuriswere decreased in population from 1.07%, 0.68% and 0.39% in 2012 to 0.29%, 0.29% and 0 in 2016, respectively, and the decrease described above was also noted in non-local population with linear downward trend(χ^2=7.585, 4.162, 5.855, P<0.05). Eleven infections with enterobiasiswere identified in 2323 children aged 3-12 years by tests of anal swab, and those infections were totally migrant children. The infection rate of enterobiasis in children were decreased from 1.05% in 2012 to 0 in 2016. Conclusion The infection rate of soil-borne nematodiasis remains relatively lower in population in Jiebei village, yet disease surveillance and health education should be strengthened in migrant population.
