摘要
目的分析超声乳化联合房角分离与超声乳化联合小梁切除术治疗急性闭角型青光眼合并白内障的临床效果。方法根据治疗方式不同分为房角组(42只眼),给予患者超声乳化联合房角分离术治疗;小梁组(42只眼),给予患者超声乳化联合小梁切除术治疗。观察两组术前及术后的眼压、视力、中央前房深度、前房角宽度及并发症发生情况。结果两组治疗后1周、1个月和6个月的眼压均低于术前、视力均高于术前,差异有统计学意义(P<0.001);术后6个月中央前房深度和平均房角关闭范围房角组均高于小梁组,但差异无统计学意义(P>0.05);同组间比较,两组术后6个月中央前房深度和平均房角关闭范围均较术前改善,差异有统计学意义(P<0.05)。结论超声乳化联合房角分离术与联合小梁切除术均是治疗急性闭角型青光眼合并白内障安全有效的方法。
Objective The objective of this study was to analyze the clinical effects of phacoemulsification combined with anterior chamber angle separation and phacoemulsification combined with trabeculectomy in the treatment of acute angle-closure glaucoma with cataract.Methods According to the different treatment methods,the patients were divided into the angle group (42 eyes),who were given phacoemulsification combined with angle separation;the trabecular group (42 eyes) were given phacoemulsification combined with trabeculectomy.The intraocular pressure,visual acuity,central anterior chamber depth,anterior chamber angle width and complication rate were observed before and after surgery.Results The intraocular pressure and visual acuity of the two groups were lower than that of the preoperative one week,one month and six months after treatment,the difference was statistically significant (P<0.001);The depth of central anterior chamber and the average range of angle closure were higher in the angle group than in the trabecular group,but the difference was not statistically significant (P>0.05);Compared with the same group,the central anterior chamber depth and average angle closure range of the two groups were improved after 6 months,the difference was statistically significant (P<0.05).Conclusions Phacoemulsification combined with angle separation and trabeculectomy are safe and effective methods for the treatment of acute angle-closure glaucoma with cataract.
