摘要
髋部骨折是老年人最常见的骨折类型,其发病率随着年龄的增长而增加,严重影响老年人日常生活和活动。目前,老年人髋部骨折的治疗方法仍然以手术治疗结合功能锻炼为主,手术治疗具有并发症少、死亡率低、恢复快等特点,但老年髋部骨折患者术后4~12个月髋关节功能不能完全恢复到骨折前水平,将对患者的日常生活质量造成影响。因此,充分了解影响老年髋部骨折患者术后功能恢复的相关因素,对改善老年髋部骨折患者的预后具有重要的临床意义。本文从年龄、性别、伤前基础病、骨折前巴塞尔(Barthel)指数、骨折类型、手术时机、手术方式、ASA分级、麻醉方式、术后并发症等方面对老年人髋部骨折术后功能恢复的影响作一综述,旨在为促进术后功能恢复提供参考。
Hip fractures are the most common type of fractures in the elderly,and their incidence increases with age,which seriously affects the daily life and activities of the elderly.At present,the treatment of hip fractures in the elderly is still mainly based on surgery combined with functional exercise.Surgical treatment is characterized by fewer complications,low mortality,and rapid recovery.However,in the elderly patients with hip fracture,hip function cannot be fully restored to the pre-fracture level 4 to 12 months after surgery,which will affect the quality of daily life of patients.Therefore,it is of great clinical significance to fully understand the relevant factors that affect the postoperative functional recovery of elderly hip fracture patients.This article focuses on the functional recovery of hip fractures in the elderly from the aspects of age,gender,and pre-injury basic disease,Barthel index before fracture,fracture type,surgical timing,surgical method,ASA classification,anesthesia method,and postoperative complications.This article reviews the impacts of previous studies;whichpreviously provided a reference for promoting patient functional recovery.
