摘要
目的探讨可溶性生长刺激表达因子-2(sST2)在不同类型心肌梗死中的早期诊断价值。方法采用酶联免疫吸附试验(ELISA)法检测93例早期心肌梗死患者及64例对照者血清中sST2浓度,比较不同类型心肌梗死患者血清sST2水平差异,同时采用受试者工作特性曲线(ROC)分析血清sST2对心肌梗死的诊断性能。结果病例组sST2水平高于对照组(t=11.96,P<0.01),其中,ST段抬高型心肌梗死(STEMI)组患者sST2水平高于非ST段抬高型心肌梗死(NSTEMI)组患者(t=2.36,P<0.05)。血清sST2对急性心肌梗死的诊断价值良好(Z=17.80,P<0.01),曲线下面积(AUC)为0.908[95%CI(0.852,0.949)]。sST2区分急性心肌梗死患者及非急性心肌梗死患者的最佳临界值(cut-off)为146.02 pg/mL,诊断敏感度为83.87%,特异度为85.94%,约登指数(YI)为0.6981。结论急性心肌梗死患者血清sST2水平升高,STEMI患者升高更明显;血清sST2对急性心肌梗死早期诊断价值良好。
Objective To explore the application of serum soluble growth stimulating express gene 2(sST2)in patients with acute myocardial infarction(AMI).Methods Serum sST2 in 93 patients with AMI and 64 controls were measured by enzyme-linked immunosorbent assay(ELISA)and compared between groups.Receiver operation curve(ROC)was analyzed to calculate diagnostic accuracy.Results Level of sST2 was significantly higher in AMI group than control group(t=11.96,P<0.01).Thereinto,level of sST2 was higher in ST segmentelevate myocardial infarction(STEMI)group than in non-STEMI(NSTEMI)group(t=2.36,P<0.05).ROC curve showed that the serum sST2 had good diagnostic value in AMI(Z=17.80,P<0.01).The area under the ROC was 0.908[95%CI(0.852,0.949)].The sensitivity,specificity and Youden index(YI)of sST2 were 83.87%,85.94%and 0.6981,respectively,when the optimal cut-off value was at 146.02 pg/mL.Conclusion The serum sST2 level was increased in patients with AMI,especially in patients with STEMI.Serum sST2 can serve as a marker in early diagnosis of AMI.
