摘要
目的观察口服仙灵骨葆胶囊联合髓关节腔灌注丹红注射液、玻璃酸钠注射液治疗早中期股骨头坏死的临床疗效。方法回顾性分析2016年7月一2017年8月医治的65例早中期股骨头坏死患者,对照组(33例)口服仙灵骨葆胶囊,治疗组(32例)在对照组的基础上加用髓关节腔灌注丹红注射液、玻璃酸钠注射液。疗效评估采用临床评定及影像学评定相结合的方法。临床评定依据Harris评分法,对治疗后患者的疼痛程度、关节畸形、关节活动度、关节功能进行评分,评定优良率;观察X线片评定股骨头影像学进展情况。观察治疗期间患者生化指标及药物不良反应发生情况。结果治疗组Harris评分的优良率高于对照组,差异有统计学意义(P<0.05);治疗组在改善疼痛程度、关节畸形、关节活动度及Harris总评分方面优于对照组,差异有统计学意义(PV0.05);影像学资料表明,治疗组影像稳定率高于对照组,差异有统计学意义(PV0.05)。结论中西医结合补益肝肾、活血通络及修复软骨法治疗早中期股骨头坏死疗效显著且不良反应少。
Objective To evaluate clinical efficacy of early and medium stage of osteonecrosis of the femoral head with Xianling Gubao capsule oral combined with hip joint cavity perfusion of Danhong and Sodium Hyaluronate injection.Methods From July 2016 to August 2017,65 patients with early and medium stage of osteonecrosis of the femoral head were retrospectively reviewed,the control group(33 cases)took orally Xianling Gubao capsule,and the treatment group(32 cases)was given Danhong and Sodium Hyaluronate injection in the hip joint cavity besides Xianling Gubao capsule.The combination of clinical evaluation and radiological assessment is used in the evaluation of curative effect.According to the Harris scoring system,the degree of pain,joint deformity,joint mobility and joint function were scored after treatment,and the excellent and good rate was evaluated.X-ray films were used to evaluate the imaging progress of femoral head and record the occurrence of adverse drug reactions during treatment.Results The excellent rate of Harris score in the treatment group was higher than that in the control group,and the difference was statistically significant(P<0.05).The treatment group was superior to the control group in improving pain,joint deformity,joint mobility,and Harris total score(P<0.05).Imaging data showed that the image stability rate of the treatment group was higher than that of the control group(P<0.05).Conclusion The method combining traditional Chinese and Western medicine, tonifying liver and kidney, activating blood circulation and repairingcartilage, are effective in treating early and medium stage of osteonecrosis of the femoral head and less adversereactions.
