摘要
目的:探讨疏肝益气汤结合赛治治疗甲状腺功能亢进的疗效及对甲状腺上动脉血流动力学的改善作用。方法:选择医院门诊就诊的甲状腺功能亢进患者98例,根据随机表格法将患者分为研究组和对照组,每组49例。对照组患者采用赛治治疗,口服,1次/d。研究组在对照组治疗的基础上,口服疏肝益气汤治疗。连续治疗6周。比较两组治疗前后中医积分、治疗疗效,比较两组治疗前后甲状腺体积变化,比较两组治疗前后甲状腺功能情况和血流动力学变化;比较两组治疗过程中的不良反应和复发情况。结果:研究组治疗总有效率为91.84%高于对照组治疗总有效率为75.51%(P<0.05);研究组治疗后中医症候积分低于对照组(P<0.05);研究组治疗后甲状腺体积小于对照组(P<0.05);研究组治疗后甲状腺指标FT3、FT和TRAb水平低于对照组,TSH水平高于对照组(P<0.05);研究组治疗后Vmax水平高于对照组, Vmin水平低于对照组(P<0.05);两组治疗过程中发生不良反应的人数比较差异无统计学意义(P>0.05)。经过6个月的随访,研究组患者中复发率低于对照组(P<0.05)。结论:疏肝益气汤治疗甲状腺功能亢进疗效确切,可有效的改善患者的临床症状,减低中医症候积分,改善患者的甲状腺功能和血流动力学指标,和赛治联合应用,可提高治疗疗效,并且安全性好,对患者的预后和病情的转归有促进作用,值得临床推广应用。
Objective:To explore the curative effect of Shugan Yiqi Decoction combined with Saizhi on treating hyperthyroidism and improving the hemodynamics of superior thyroid artery.Methods:A total of 98 patients with hyperthyroidism were selected and divided into study group and control group according to random table method,49 cases in each group.The patients in the control group were treated with methimazole,orally,once a day.The study group was treated with Shugan Yiqi Decoction on the basis of the control group.The treatment lasted for 6 weeks.We compared the two groups TCM integrals,treatment efficacy,thyroid volume changes,thyroid function and hemodynamic changesbefore and after treatment and observed the adverse reactions and recurrence.Results:The total effective rate of the study group was 91.84%which was higher than that of the control group(75.51%)(P<0.05).The score of TCM symptoms in the study group was lower than that of the control group(P<0.05).The thyroid volume in the study group was smaller than that in the control group(P<0.05).The levels of FT3,FT and TRAb in the study group were lower than those in the control group,and the levels of TSH in the study group were higher than those in the control group(P<0.05).After treatment,the Vmax level in the study group was higher than that in the control group,and the Vmin level was lower than that in the control group(P<0.05).There was no significant difference in the number of adverse reactions between the two groups(P>0.05).After 6 monthsfollow-up,the recurrence rate in the study group was lower than that in the control group(P<0.05).Conclusion:Shugan Yiqi Decoction is effective on treating hyperthyroidism.It can effectively improve clinical symptoms,reduce TCM syndrome scores,improve thyroid function and hemodynamic indexes of patients.Combined with Saizhi treatment,it can improve the therapeutic effectand has good safety.It can promote the prognosis.It is worthy of clinical application.
