摘要
目的观察自拟君子消痞汤治疗气阴两虚型糖尿病胃轻瘫患者的临床疗效及对胃排空能力及相关指标的影响。方法将2016年1月—2017年12月沧州中西医结合医院诊治的100例糖尿病胃轻瘫患者随机分为治疗组和对照组各50例。治疗组在一般治疗基础上采用自拟君子消痞汤治疗,对照组在一般治疗基础上给予甲钴胺静滴联合莫沙必利口服,2组均以2周为1个疗程。观察2组患者治疗前后临床症状积分、胃排空率以及血浆胃动素(MLT)、胆囊收缩素(CCK)水平变化情况,评价比较2组临床疗效。结果治疗1个疗程后,对照组疲乏无力、五心烦热症状积分基本无改善,治疗组疲乏无力、腹胀早饱、恶心呕吐、五心烦热积分均明显低于治疗前及对照组(P均<0.05);治疗组临床愈显率为86.0%(43/50),对照组为50.0%(25/50),治疗组显著高于对照组(P<0.05);治疗后2组胃排空率均较治疗前明显提高(P均<0.05),且治疗组较对照组提高更明显(P<0.05);治疗组患者治疗后MLT、CCK水平较治疗前、同期对照组明显降低(P均<0.05)。结论自拟君子消痞汤治疗气阴两虚型糖尿病胃轻瘫患者效果理想,可有效改善临床症状,促进胃排空,可能与调节MLT、CCK水平有关。
Objective It is to observe the clinical efficacy of self-made Junzi Xiaopi decoction on diabetic gastroparesis of type of Qi-Yin deficiency and its effect on gastric emptying capacity and related indicators.Methods 100 patients with diabetic gastroparesis treated in Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine from January 2016 to December 2017 were randomly divided into treatment group and control group,50 cases in each group.The treatment group was treated with self-made Junzi Xiaopi decoction on the basis of general treatment,and the control group was treated with mecobalamin by intravenous drip and mosapride by oral administration on the basis of general treatment.Both groups were treated for 2 weeks as one treatment course.The changes of clinical symptom scores,gastric emptying rate,and levels of plasma motilin(MLT)and cholecystokinin(CCK)before and after treatment were observed,the clinical curative effects were evaluated in the two groups.Results After one course of treatment,the scores of fatigue and burning sensation of five centres had no improvement in the control group,but the scores of fatigue,abdominal fullness,nausea and vomiting,and burning sensation of five centres in the treatment group were significantly lower than those before treatment and in the control group(P<0.05).The clinical cure rate was 86.0%(43/50)in the treatment group and 50.0%(25/50)in the control group,the rate in the treatment group was significantly higher than that in the control group(P<0.05).After treatment,the gastric emptying rates of the two groups were significantly higher than that before treatment(P<0.05).and the rates in the treatment group was significantly higher than that in the control group(P<0.05).After treatment,the levels of MLT,CCK in the treatment group were significantly lower than those before treatment and in the control group(P<0.05).Conclusion Self-made Junzi Xiaozhi decoction which can effectively improve clinical symptoms and promote gastric emptying has good curative effec
