摘要
目的:探讨柴芍承气汤对不同时期高脂血症性急性胰腺炎患者淀粉酶含量、血脂及炎症因子等的影响。方法:80例确诊为高脂血症性急性胰腺炎患者,随机分为对照组(20例)、早期(发病48 h内)中药干预治疗组20例、中期(发病72 h内)中药干预治疗组20例、晚期(发病72 h后)中药干预治疗组20例。对照组以西医常规治疗,治疗组加服用柴芍承气汤,分别在住院第3、5、7天及14天观测患者血淀粉酶(SAMY)、尿淀粉酶(UAMY)、白细胞(WBC)、C反应蛋白(CRP)、三酰甘油(TG)、肿瘤坏死因子(TNF-α)、白细胞介素-6(IL-6)和白细胞介素-10(IL-10)水平变化。结果:各治疗组患者住院后TG、SAMY、UAMY及CRP水平均显著降低(P<0.05),且早期中药干预治疗组效果优于中期和晚期;早期和中期中药干预治疗组在住院后TNF-α和IL-6水平显著降低(P<0.05),IL-10水平显著上调(P<0.05),晚期中药干预治疗组IL-6和IL-10水平有显著性差异(P<0.05)。结论:柴芍承气汤可改善高脂血症性急性胰腺炎血脂水平,有效下调炎症介质表达。
Objective:To study the effects of Chaishao Chengqi Decoction on amylase,blood lipids and inflammatory cytokines in hyperlipdemic acute pancreatitis patients at different periods.Methods:Totally 80 cases of hyperlipidemic acute pancreatitis patients were selected and divided into five groups according to the random number table,with 20 in control group and 60 cases in clinical groups.Clinical groups received Dachengqi decoction(enema once a day,oral twice a day),which were divided into 20 cases of prior period patients(within 48 hours of the onset of illness),20 cases of middle period patients(within 72 hours of the onset of illness),20 cases of last period patients(72 hours after onset).The level of SAMY,UAMY,WBC,CRP,TG,TNF-α,IL-6,IL-10 were detected after treating 3 days,5 days,7 days,14 days respectively.Results:The levels of TG,SAMY,UAMY,CRP of patients decreased significantly after treatment(P<0.05).Moreover,the effects of prior period patients were better than those of the middle and the last.There was a great decrease in the levels of TNF-αand IL-6 in prior and middle period patients(P<0.05),while increased in the level of IL-10(P<0.05).Compared with control group,the levels of IL-6 and IL-10 was greatly different(P<0.05).Conclusion:Chaishao Chengqi Decoction can significantly reduce TG level and the expressions of serum inflammatory cytokines.
