摘要
目的探究空心钉内固定与微型钢板内固定治疗后踝大块骨折的临床效果。方法选择我院2016年3月~2018年3月收治的77例后踝大块骨折患者为研究对象,根据患者入院住院病床尾号单双号不同分为对照组(38例)与观察组(39例)。对照组开展空心钉内固定治疗,观察组开展微型钢板内固定治疗,比较两组患者术中、术后情况,对所有患者进行为期1年的随访,评价骨折完全愈合时间、完全负重时间及踝关节功能情况。结果观察组患者的术中出血量少于对照组,差异有统计学意义(P<0.05)。两组患者的手术时间和术后并发症总发生率比较,差异无统计学意义(P>0.05)。观察组患者的骨折完全愈合时间和完全负重时间均短于对照组,差异有统计学意义(P<0.05)。观察组患者的治疗优良率和美国矫形外科足踝协会(AOFAS)评分高于对照组,差异均有统计学意义(P<0.05)。结论空心钉内固定与微型钢板内固定治疗后踝大块骨折均疗效肯定,但微型钢板内固定治疗可减少术中出血量,加快骨折愈合速度,改善踝关节功能,应用推广价值大。
Objective To explore the clinical effect of internal fixation with hollow screw and mini-plate in the treatment of massive ankle fracture.Methods A total of 77 patients with massive posterior ankle fracture admitted to our hospital from March 2016 to March 2018 were selected as the research objects.According to the even and odd number of the patient's admission bed,they were divided into the control group(38 cases)and the observation group(39 cases).The patients in the control group were treated with hollow screw internal fixation,while those in the observation group were treated with mini-plate internal fixation.The intraoperative and postoperative conditions of the two groups were compared.All the patients were followed up for one year to evaluate the fracture complete healing time,complete weight bearing time and ankle joint function.Results The intraoperative blood loss of patients in observation group was less than that in the control group,the difference was statistically significant(P<0.05).There were no significant difference between the two groups in operation time and total postoperative complication rate(P>0.05).The fracture healing time and full load completely time in observation group were shorter than those in the control group,the differences were statistically significant(P<0.05).The treatment excellent rate and American orthopedic foot and ankle society(AOFAS)score in the observation group were higher than those in the control group,with statistically significant differences(P<0.05).Conclusion Both internal fixation with hollow screw and mini-plate have definite therapeutic effects on massive fracture,but mini-plate internal fixation treatment can decrease the intraoperative blood loss,accelerate fracture healing,improve ankle function,with great application promotion value.
