摘要
目的了解中医药领域患者价值观及偏好的研究现状。方法计算机检索中、英文医学文献数据库,检索时间为建库至2019年3月4日,并手工检索近10年出版的循证医学专著。采用定性与定量分析相结合的方法对纳入文献的发表年份、作者地区、研究目的、研究设计类型、中医药干预措施、研究对象疾病类型进行统计分析,数据以百分比表示。结果共纳入116篇文献,其中中文文献23篇,英文文献93篇。研究内容包括患者的治疗意愿、就医意愿、治疗满意度、自我管理、临床决策、以患者为中心6类。包括中药、针灸等4种中医药干预措施,纳入研究对象以不限定疾病类型的患者群体为主。设计类型包含定量研究、定性研究及混合研究设计。已有研究将患者价值观及偏好作为研究的一部分,如改良临床研究设计、制定临床实践指南、核心指标集的形成等。结论中医药患者价值观及偏好在临床研究中的地位逐渐受到重视,目前以慢性病患者的治疗意愿为主要研究内容,多采用横断面研究及定性研究。现有研究对患者价值观及偏好的概念较为模糊,缺少科学研究测评工具,尚需进一步研究。
Objective To understand the research status of patients′values and preferences in the field of traditional Chinese medicine(TCM).Methods The Chinese and English medical literature databases were searched.The literature time was from the establishment of the database to March 4,2019.The evidence-based medical monographs published in recent 10 years were manually searched.A qualitative and quantitative analysis were used to statistically analyze the publication year,author area,research purpose,type of research design,TCM interventions,and type of disease of the study subject.The data were expressed as percentages.Results A total of 116 papers were included,of which 23 were Chinese and 93 were English.The research contents included 6 types such as patients′willingness to treat,willingness to seek medical treatment,treatment satisfaction,self-management,clinical decision-making,and patient-centeredness,and 4 types of TCM intervention measures including Chinese herbal medicine and acupuncture were researched.The study subjects were mainly composed of patients with unlimited disease types.Design types included quantitative research,qualitative research,and mixed research designs.Existing research took patient values and preferences as part of research,such as improving clinical research design,formulating clinical practice guidelines,and forming core index sets.Conclusion The status of TCM patients′values and preferences in clinical research has gradually been paid more attention.At present,the treatment content of patients with chronic diseases is the main research content,and cross-sectional and qualitative research are mostly used.Existing research has a vague concept of patient values and preferences,and lacks scientific research assessment tools.Therefore,further research is needed.
