摘要
目的:分析宜昌市葛洲坝地区结肠镜受检者结直肠病变情况。方法:对2015年6月~2018年7月在我院行结肠镜检查的2201例门诊及住院患者进行回顾性分析。结果:2201例受检者中共检出肠道病变1794例(81.51%),其中结直肠息肉、结直肠炎合并息肉及结直肠癌的检出率男性高于女性,结直肠炎的检出率女性高于男性,差异均有统计学意义(均P<0.05)。以腹痛、便秘、便血、腹泻及腹部包块为主要临床表现的患者结直肠病变检出率较高,常见病变为结直肠息肉、结直肠炎和结直肠癌。老年组结直肠癌的检出率显著高于中年组和青年组(均P<0.05),且老年受检组中,男性结直肠癌检出率显著高于女性,差异有统计学意义(P<0.05)。结论:葛洲坝地区有肠道相关临床症状患者的结直肠疾病检出率较高,中年及以上人群应重视结肠镜检查,以早期发现结直肠病变,尤其是结直肠癌及癌前病变。
Objective:To analyze the colorectal lesions of colonoscopy subjects in Gezhouba area of Yichang.Methods:A retrospective analysis was conducted on 2201 patients underwent colonoscopy in our hospital from June 2015 to July 2018.Results:There were 1794 cases(81.51%)of intestinal lesions detected out.The detection rate of colorectal polyps,and proctitis complicated with polyps,and colorectal cancer were all higher in men,while proctitis was higher in women(all P<0.05).Patients with abdominal pain,constipation,hematochezia,diarrhea and abdominal mass as the main clinical manifestations had higher detection rate,and the common diseases were colorectal polyps,proctitis and colorectal cancer.Colorectal cancer in the elderly group was significantly higher than that in the middle-age and the youth group(all P<0.05).Furthermore,man had higher detection rate than women in the elderly group(P<0.05).Conclusion:The detection rate of colorectal diseases is relatively high in patients with intestinal symptoms in Gezhouba area.And the middle-aged and above population should pay attention to colonoscopy to detect colorectal lesions,especially colorectal cancer and precancerous lesions.
