摘要
目的探讨高龄经产孕妇对孕期围产期健康知识的知晓状况及其影响因素,为高龄经产孕妇提供针对性的健康知识教育提供依据。方法采用横断面研究方法,通过便利抽样选取本医院妇产科门诊登记产检的高龄经产孕妇302例,采用自行编制的"高龄经产孕妇孕期围产期健康知识调查表"进行调查,分析高龄经产孕妇孕期围产期健康知识的知晓状况及其影响因素。结果回收263份有效问卷,263例高龄经产孕妇孕期围产期健康知识总分(33.53±5.45)分,得分率67.06%;其中良好38例,占14.45%,良好以下225例,占86.55%。高龄经产孕妇孕期围产期健康知识4个维度在合格及以上占比由高至低依次为:新生儿护理知识210例(79.85%)、妊娠期知识184例(69.96%)、产时配合知识155例(58.94%)、产后康复知识117例(44.49%)。以知识掌握是否良好,采用二元Logistic回归分析高龄经产孕妇孕期围产期健康知识知晓影响因素显示:年龄、文化程度及居住地是高龄经产孕妇孕期围产期健康知识知晓情况的主要影响因素(均P<0.001)。结论高龄经产孕妇对孕期围产期健康知识的知晓情况不容乐观,特别是年龄越大、文化程度越低、居住在乡村者其孕期围产期健康知识知晓情况越差。医务人员应积极开展针对高龄经产孕妇孕期围产期健康知识的宣传和普及工作,尤其要加大对年龄在40岁及以上、高中及以下学历及居住在乡村的高龄经产妇在医疗、教育资源的投入,切实提高高龄经产孕妇孕期围产保健水平,促进母婴健康。
Objective To explore the awareness of perinatal health knowledge among old women with menstruation and the influencing factors,and provide evidence for targeted perinatal health education for old women with menstruation.Methods Using cross-sectional method and random sampling,302 old multipartum pregnant women registered in the department of obstetrics and gynecology in our hospital were engaged in the study.A self-compiled questionnaire on perinatal health knowledge of old multipartum pregnant women was used to investigate the awareness of perinatal health knowledge of the old multipartum pregnant women.Results 263 valid questionnaires were retrieved.The average score of perinatal health knowledge of 263 elderly multiparous pregnant women during pregnancy was(33.53±5.45),with a scoring rate of 67.06%.Among them,38 were good,accounting for 14.45%,and 225 were bad,accounting for 86.55%.The four dimensions of perinatal health knowledge scored qualified and above included neonatal nursing knowledge(n=210,79.85%),pregnancy knowledge(n=184,69.96%),intrapartum cooperation knowledge(n=155,58.94%)and postpartum rehabilitation knowledge(n=117,44.49%)in a descending order.The logistic regression showed that age,education level and residence were the main influencing factors of perinatal health knowledge(P<0.001).Conclusions The awareness of perinatal health knowledge among the elderly women with menstruation is unsatisfactory,especially among the elderly.The lower their education level is,the worse the knowledge of perinatal health among those living in rural areas.Medical staff should actively carry out propaganda and popularization of perinatal health knowledge for older women with menopause,especially increasing their investment in medical and educational resources for those aged more than 40 years and with lower education level and those living in rural areas,so as to effectively improve their health level for older women with menopauset to promote maternal and infant health.
