摘要
目的:探讨经肛门全直肠系膜切除术(TaTME)和腹腔镜全直肠系膜切除术(LapTME)治疗中低位直肠癌的临床效果与安全性比较。方法:选取2015年1月—2018年11月在淮安市洪泽区人民医院诊治的中低位直肠癌患者90例进行前瞻性研究,采用随机数字法分为TaTME组(45例)和LapTME组(45例)。观察并记录术中及术后相关指标。结果:两组手术时间和术中出血量相比差异均无统计学意义(P>0.05);与LapTME组相比,TaTME组患者的术后恢复进食时间、术后首次下床活动时间及住院时间均显著缩短,保肛率显著升高,差异均有统计学意义(P<0.05),TaTME组术后并发症的总发生率为8.9%,LapTME组为13.3%,两组差异无统计学意义(P>0.05)。结论:相比于LapTME,TaTME治疗中低位直肠癌安全可靠且可显著加快患者的术后恢复。
Objective:To compare the clinical efficacy and safety of transanal total mesorec-tal excision(TaTME)and laparoscopic total mesorectal excision(LapTME)in the treatment of middle and low rectal cancer.Methods:A prospective study was conducted on 90 patients with middle and low rectal cancer treated in Hongze District People's Hospital of Huai'an City from January 2015 to November 2018.All patients were randomly divided into TaTME group(45 cases)and LapTME group(45 cases).Relevant indexes during and after operation were observed and recorded.Results:There was no significant difference in operation time and intraoperative bleeding volume between the two groups(P>0.05).Compared with LapTME group,the recovery eating time,the first time out of bed and hospital stay in TaTME group were significantly shortened,and the anus-preserving rate was significantly increased(P<0.05).The total incidence of postoperative complications was 8.9%in LapTME group and 13.3%in LapTME group.There was no significant difference between the two groups(P>0.05).Conclusion:Compared with laparoscopic total mesorectal excision,transanal total mesorectal excision is safe and reliable in the treatment of middle and low rectal cancer,and can significantly accelerate the recovery of patients.
