摘要
目的探讨标准统一且可复制的回弹型体位垫在ERCP术中的应用效果。方法选取2019年1月至2019年6月期间320例行ERCP诊疗患者,随机分为对照组和实验组各160例。对照组患者行ERCP术中采用传统的体位安置法,而实验组患者采用自制记忆回弹型体位垫安置术中体位。比较护士在安置2组患者时所需时间、术中躁动、体位垫滑脱有无重复安置体位的情况、侧俯卧位身体受压局部有无皮肤发红、麻木感等进行比较。患者ERCP术中舒适度采用Kolcaba的舒适状况量表(CCQ) Likert4级评分法,包括生理、心理、精神、社会文化和环境4个维度,共28项。结果实验组护士体位安置时间、术中躁动、体位垫滑脱,侧俯卧位身体受压局部有无皮肤发红、麻木感等明显优于对照组,差异有统计学意义(P <0.05)。2组患者ERCP术中CCQ各维度及总分比较,实验组得分明显高于对照组,差异有统计学意义(P <0.01)。结论 ERCP术中采用回弹型体位垫可以节省护士手术的安置时间,术中无重复安置体位的情况发生,可提高患者的术中的舒适度及满意度,改善皮肤受压的情况,提高与医生的配合度。该体位垫设计科学、合理、简单、方便,适合于需要安置侧俯卧的手术患者,可按设计复制便于统一标准,值得临床推广应用。
Objective To study the application effect of standard uniform and replicable rebound body pad in ERCP. Methods From January 2019 to June 2019, 320 ERCP patients were randomly and equally divided into the control group and experimental group. In the control group, the traditional placement method was used in ERCP, while in the experimental group, the self-made memory rebound body pad was used in the placement procedure. We compared the time required by nurses to place two groups of patients, the restlessness in surgery,the condition that the body cushion slips out of the body position with or without repeated placement, and the presence of skin redness and numbness in the pressure area of the body in the side prone position. The patient’s comfort level in ERCP was based on Kolcaba’s Comfort Scale(CCQ) Likert 4 rating method, which included four dimensions: physiological,psychological, spiritual, sociocultural, and environmental, for a total of 28 items.Results In the experimental group, the nurse’s placement time, intraoperative restlessness, body position slip, lateral prone body pressure, whether there is skin redness, numbness, etc. were significantly better than those in the control group, and the difference was statistically significant(P <0.05). Comparison of CCQ dimensions and total score between the two groups of patients in ERCP surgery showed that the score of the experimental group was significantly higher than that of the control group, and the difference was statistically significant(P < 0.01).Conclusio The use of rebound body pads in ERCP surgery can save the placement time of nurses. The occurrence of no repeated placement in surgery can improve the patient’s comfort and satisfaction in surgery and improve skin pressure. Improve cooperation with doctors. It is scientifically, reasonably, simply and conveniently designed for patients who need to be placed on the side of the prostrate.
