摘要
目的比较不同剂量右美托咪定分别复合布托啡诺应用于剖宫产术后镇痛中的临床效果。方法选取2017年1月~2018年10月在我院妇产科就诊的接受剖宫产手术的足月产妇226例,根据随机数字表法将其分为对照组(113例)和观察组(113例)。两组均采用硬膜外麻醉,术后应用静脉自控镇痛。对照组给予200μg右美托咪定复合10 mg布托啡诺镇痛,观察组给予300μg右美托咪定复合10 mg布托啡诺镇痛。观察两组术后1、2、6、12、24、48 h的镇痛、镇静效果以及48 h内不良反应发生情况。结果两组产妇术后不同时间点的疼痛视觉模拟量表(VAS)评分和Ramesay镇静效果评分比较,差异均无统计学意义(P>0.05)。观察组不良反应总发生率低于对照组,差异有统计学意义(11.50%vs. 24.78%,P<0.05)。结论应用300μg右美托咪定复合10 mg布托啡诺可以发挥较强的镇痛镇静效果,且不良反应发生率较低,在剖宫产术后自控静脉镇痛中能达到满意的镇痛效果。
Objective To compare the clinical effect of different doses of Dexmedetomidine combined with Butorphanol on analgesia after cesarean operation. Methods A total of 226 full-term pregnant women undergoing cesarean operation in the department of obstetrics and gynecology of our hospital from January 2017 to October 2018 were divided into control group(113 cases) and observation group(113 cases) according to the random number table method. Epidural anesthesia was used in both groups, and patient-controlled intravenous analgesia was used after operation. The control group was given 200 μg Dexmedetomidine combined with 10 mg Butorphanol for analgesia, while the observation group was given 300 μg Dexmedetomidine combined with 10 mg Butorphanol for analgesia. The analgesic and sedative effects of 1、2、6、12、24、48 hours after operation and adverse reactions within 48 hours after operation were observed in the two groups. Results There were no significant differences in visual analogue scale(VAS) score and Ramesay sedation effect score between the two groups at different time points after operation(P>0.05). The total incidence of adverse reactions in the observation group was lower than that in the control group(11.50% vs. 24.78%, P<0.05). Conclusion To use 300 μg Dexmedetomidine combined with 10 mg Butorphanol can exert a strong analgesic and sedative effect, and the incidence of adverse reactions is low. It can achieve satisfactory analgesic effect in patient-controlled intravenous analgesia after cesarean section.
