摘要
[目的]系统评价运动干预对肺癌病人癌因性疲乏及生活质量的影响。[方法]通过计算机检索the Cochrane Library、EMbase、PubMed、OVID、Web of Science、中国知网、中国生物医学文献数据库、维普数据库和万方数据库中关于运动干预对肺癌病人癌因性疲乏及生活质量影响的随机对照试验,检索时间为建库至2018年9月14日。由2名研究者根据纳入与排除标准独立筛选文献、提取资料和评价质量后,采用RevMan 5.3软件进行Meta分析。[结果]最终纳入7项随机对照试验。Meta分析结果显示:与对照组相比,试验组病人生活质量提高[MD=4.53,95%CI(0.29,8.77),P=0.04],但病人疲乏症状改善不明显[SMD=-0.22,95%CI(-0.93,0.49),P=0.54]。结局指标为疲乏的研究亚组分析结果显示:肺癌病人的治疗方式可能是研究间的异质性来源。[结论]现有证据表明:运动干预能够改善肺癌病人生活质量,但对疲乏症状改善效果尚不明确。受纳入研究文献的数量所限,上述结论需进一步开展多项经严格设计的随机双盲对照试验加以验证。
Objective:To systematically evaluate effects of exercise intervention on cancer-related fatigue and quality of life in patients with lung cancer.Methods:Randomized controlled trials on effects of exercise intervention on cancer-related fatigue and quality of life in patients with lung cancer were retrieved from the Cochrane Library,EMbase,PubMed,OVID,Web of Science,CNKI,CBM,VIP Database,WanFang Database.The retrieval time was from the inception to September 14,2018.Literatures were screened independently by two researchers according to inclusion and exclusion criteria.After extracting data and evaluating included literatures,Revman 5.3 software was used for taking Meta-analysis.Results:Finally,7 randomized controlled trials were included.Meta-analysis showed that compared with the control group,quality of life of patients in experimental group had been significantly improved[MD=4.53,95%CI(0.29,8.77),P=0.04].However improvement of fatigue symptoms hadn′t been significant[SMD=-0.22,95%CI(-0.93,0.49),P=0.54].Subgroup analysis with fatigue as outcome index showed that different treatment methods of lung cancer patients may be the source of heterogeneity among included studies.Conclusions:Current evidence shows that exercise intervention can improve the quality of life of lung cancer patients.Nevertheless effect of exercise on alleviating fatigue symptoms is not clear.Due to the limitation of number of included literatures,the above conclusions need to be further verified through conducting multiple strictly designed randomized double-blind controlled trials.
