摘要
目的:观察中药塌渍热疗治疗风寒湿痹型膝关节骨性关节炎的临床疗效。方法将95例患者按就诊顺序随机分为观察组48例对照组47例,对照组给予常规西医治疗方法,观察组在对照组治疗的基础上给予中药塌渍热疗法2组均连续治疗30天,比较2组总有效率及治疗前后关节液各指标水平、临床症状积分、膝关节功能评分等变化情况。结果总有效率观察组为91.67%(44/48),对照组为76.60%(36/47),2组比较差异有统计学意义(P<0.05)。临床症状积分、膝关节功能评分治疗前后2组组内比较,差异有统计学意义(P<0.05);治疗后2组组间比较差异也有统计学意义(P<0.05)。膝关节关节液中胰岛素样生长因子1(IGF-1)、骨钙素(BGP)、骨保护素(CPG)、核因子-κβ受体活化因子配体(RANCL)水平治疗前后2组组内比较,差异有统计学意义(P<0.05);治疗后组间比较差异也有统计学意义(P<0.05)。结论:中药塌渍热疗治疗风寒湿痹型膝关节骨性关节炎的效果显著,可显著改善关节液各指标水平,且对临床症状和膝关节功能的恢复具有促进作用。
Objective:To observe clinical effects of herbal hot compressing in the treatment for knee osteoarthritis(KOA) of wincd-colcl-dampness arthralgia pattern.Methods:All 95 patients were randomized into 48 cases of the observation group and 47 case of the control group according to the visiting order,the control group accepted conventional Western medicine therapy,and the observation group herbal hot compressing,both groups were treated for 30 days consecutively,to compare total effective rates,the levels of different indexes in joint fluid,clinical symptom scores,knee joint functional scores between both groups.Results:Total effective rate of the observation group was 91.67%(44/48),higher than 76.60% of the control group(36/47),and the difference showed statistical meaning(P<0.05).The difference could be found in clinical symptom scores and knee joint functional scores within two groups before and after treating(P<0.05);the difference was statistically significant between both groups after treating(P<0.05).The difference showed statistical meaning in IGF-1,BGP,OPG and RANKL in knee joint fluid within two groups before and after treating(P<0.05);the difference showed statistical meaning between both groups after treating(P<0.05).Conclusion:Herbal hot compressing is effective in the treatment for KOA of wind-cold-dampness arthralgia pattern,and it could significantly improve different indexes levels in joint fluid,promote the recovery of clinical symptoms and knee joint function.
