摘要
目的探索提高儿童专科医院普通内科门诊抗菌药物规范使用率的管理办法。方法制定抗菌药物使用的管理制度,向临床医务人员提供抗菌药物信息,定期公布临床标本送检率、致病菌分布及药敏统计结果,对临床医生进行抗菌药物相关知识的培训等,建立符合当下的儿科门诊抗菌药物规范使用管理方案,以指导临床合理用药,加强监督检查、反馈及责任追究。结果通过此次质量改进项目“提高普通内科门诊抗菌药物规范化使用率”的开展,2017年上半年度普通内科门诊抗菌药物使用率为50.24%,比2016年上半年度的52.29%下降了2.05%;抗菌药物规范化使用率从2016年8月开始总体呈上升的趋势,截止2017年6月份已经达到14.80%;2017年上半年度普通内科门诊药占比为73.00%,较2016年上半年度的80.00%下降了7.00%;2017年上半年患者就诊均次费用为247元,较2016年上半年度的287元下降了13.94%。结论通过本次项目开展后,医务人员合理使用抗菌药物的意识增强,病原学送检率上升,抗菌药物使用率下降,抗菌药物使用逐步规范,采取的管理策略有效。
Objective To explore the management methods to improve the standardized use rate of antibacterial drugs in general medical clinics of children's hospitals.Methods Develop a management system for the use of antibacterial drugs,provide clinical medical personnel with information on antibacterial drugs,regularly publish the clinical specimen inspection rate,pathogenic bacteria distribution and drug susceptibility statistics,and train clinicians on antibacterial drug related knowledge.The current pediatric outpatient antibacterial drugs standard use management plan to guide the rational use of drugs in the clinic,strengthen supervision and inspection,feedback and accountability.Results Through the implementation of the quality improvement project"Improving the standardized use rate of antibacterial drugs in general medical clinics",the use rate of antibacterial drugs in general medical clinics in the first half of 2017 was 50.24%,a decrease of 2.05%from 52.29%in the first half of 2016;The standardized use rate of antibacterials has been on the rise since August 2016 and has reached 14.80%as of June 2017;the proportion of general medical outpatient drugs in the first half of 2017 was 73.00%,compared with 80.00%in the first half of 2016,it decreased by 7.00%;the average cost of visiting patients in the first half of 2017 was 247 yuan,which was a decrease of 13.94%compared with 287 yuan in the first half of 2016.Conclusion After the management,the medical staff's awareness of the rational use of antibacterial drugs has increased,the rate of pathogenic examinations has increased,the use of antibacterial drugs has decreased,the use of antibacterial drugs has been gradually standardized,and the management strategy adopted is effective.
