摘要
目的探究不同剂量右美托咪定复合丙泊酚在无痛人工流产中的应用效果。方法选择2018年5月~2019年3月的无痛人工流产患者200例,随机分成5组,分别为对照组和观察A,B,C,D组,各40例。对照组应用生理盐水10mL+丙泊酚2mg/kg,观察A组给予右美托咪定0.25μg/kg+丙泊酚2mg/kg,观察B组右美托咪定0.50μg/kg+丙泊酚2mg/kg,观察C组给予右美托咪定0.75μg/kg+丙泊酚2mg/kg,观察D组右美托咪定1.0μg/kg+丙泊酚2mg/kg,比较不同剂量右美托咪定复合丙泊酚在无痛人工流产中的应用效果。结果在T0、T5、T6时,五组患者的平均动脉压(MAP)与心率(HR)比较,差异无统计学的意义(P>0.05)。在T1、T2、T3、T4时,观察组D的心率(HR)明显低于观察组A,B和对照组,差异有统计学的意义(P<0.05)。观察A组和观察B组的心率(HR)比较,差异无统计学的意义(P>0.05)。在T3、T4时,观察组A,B,C,D的平均动脉压(MAP)较对照组高,差异有统计学意义(P<0.05)。五组的SPO2(血氧饱和度)在各时间点比较,差异无统计学意义(P>0.05)。五组的手术时间比较,差异无统计学的意义(P>0.05)。观察组B,C,D的丙泊酚用量比较,差异无统计学的意义(P>0.05)。对照组和观察组A的丙泊酚用量明显高于观察B,C,D组,差异有统计学的意义(P<0.05)。观察C,D组的清醒时间较观察A,B组长,差异有统计学的意义(P<0.05)。观察A组和观察B组的清醒时间比较,差异无统计学意义(P>0.05)。对照组的注射痛明显高于观察各组,差异有统计学意义(P<0.05)。对照组和观察A组的体动反应,呼吸暂停和术后宫缩痛发生率高于观察B,C,D组,差异有统计学的意义(P<0.05)。结论应用0.50μg/kg的右美托咪定复合丙泊酚在无痛人工流产中的应用效果较好,推荐在临床中应用。
Objective To explore the application of different doses of dexmedetomidine combined with propofol in painless induced abortion.Methods 200 patients with painless induced abortion from May 2018 to March 2019 were selected and they were randomly divided into 5 groups including the control group,observation A group,observation B group,observation C group and observation D group,with 40 cases in every group.The control group was given normal saline 10mL and propofol 2mg/kg.The observation A group was given dexmedetomidine 0.25μg/kg and propofol 2mg/kg.The observation B group was given dexmedetomidine 0.50μg/kg and propofol 2mg/kg.The observation C group was given dexmedetomidine 0.75μg/kg and propofol 2mg/kg.The D observation group was given dexmedetomidine 1.0μg/kg and propofol 2mg/kg.The application effects of different doses of dexmedetomidine combined with propofol in painless induced abortion were compared.Results At T0,T5,and T6,there was no statistically significant difference in mean arterial pressure(MAP)and heart rate(HR)among the 5 groups(P>0.05).At T1,T2,T3,and T4,the heart rate(HR)of observation D group was significantly lower than that of observation A group,observation B group and the control group,and the difference was statistically significant(P<0.05).Here was no statistically significant difference in heart rate(HR)between observation A group and observation B group(P>0.05).At T3 and T4,the mean arterial pressure(MAP)of observation A groups,observation B group,observation C group and observation D group was all higher than that of the control group,and the difference was statistically significant(P<0.05).The SPO2(oxygen saturation)of the 5 groups were compared at each time point,and the difference was not statistically significant(P>0.05).There was no statistically significant difference in the operation time among the 5 groups(P>0.05).There was no statistically significant difference in the dose of propofol in observation B group,observation C group and observation D group(P>0.05).The doses of
