单免疫球蛋白-白介素1受体蛋白在原发性EB病毒感染肝损伤患儿血清中的表达及临床意义

摘要 目的探讨单免疫球蛋白-白介素1受体蛋白(SIGIRR)在原发性EB病毒感染肝损伤患儿血清中的表达及临床意义。方法选取40例原发性EB病毒感染患儿(入院时EB-IgM和EB-DNA检测均阳性)为A组,门诊体检EB-IgG阳性(EB-VCA-IgG、EB-NA-IgG)而EB-IgM阴性的40例儿童为B组,门诊体检EB-IgG和EB-IgM均阴性的40例儿童为C组。检测并比较A组入院时及治疗1周后血清SIGIRR表达水平及AST、ALT水平,比较3组对象血清SIGIRR表达水平,同时分析A组入院时及治疗1周后血清SIGIRR表达水平与ALT水平的相关性。结果治疗1周后,原发性感染患儿血清SIGIRR表达水平明显升高,ALT水平明显降低,差异均有统计学意义(均P<0.05);而AST水平变化不明显,差异无统计学意义(P>0.05)。A组入院时血清SIGIRR表达水平与B、C组的差异有统计学意义(P<0.05),治疗1周后与B、C组的差异无统计学意义(P>0.05)。原发性感染患儿入院时血清ALT水平明显升高,与血清SIGIRR表达水平呈负相关(r=-0.806,P<0.05),治疗1周后两者未见相关性(r=0.106,P>0.05)。结论EB病毒感染后患儿血清SIGIRR表达水平明显降低,与ALT水平相关,是肝损伤的保护因子。 Objective To investigate the expression and clinical significance of SIGIRR(single immunoglobulin interleukin-1 related receptor)in EB virus(EBV)infected children.Methods From January 2018 to December 2018,40 child patients with positive EB-IgM and EB-DNA(group A),40 healthy children with positive EB-IgG and negative EB-IgM(group B)and 40 healthy children with negative EB-IgG/EB-IgM(group C)were enrolled in the study.The AST and ALT levels were recorded and the serum SIGIRR level was detected by ELISA.Results Serum SIGIRR levels in group A were significantly lower than those in groups B and C before treatment(P<0.05).After treatment the SIGIRR level of group A was significantly increased,which was no significant different with those in group B and C(P>0.05).Compared to that before treatment the ALT level in group A was significantly reduced after treatment(P<0.05),but AST level was not changed.There was a negative correlation between SIGIRR and ALT in group A before treatment(r=-0.806,P<0.05),but not after treatment(r=0.106,P>0.05).Conclusion SIGIRR is decreased in the course of EBV active infection,which is correlated with the change of ALT.SIGIRR is significantly increased after treatment,suggesting that it may be a protective factor of liver injury caused by EBV infection in children.
