摘要
目的通过检测妊娠期糖尿病(GDM)患者血清、脐血、胎盘组织中Leptin的表达水平,进一步探讨Leptin在妊娠期糖尿病发生、发展中的意义。方法选取的对象为2018年3月至2019年3月在我院行剖宫产术的妊娠孕妇,共80例,根据产妇是否患有妊娠期糖尿病,分为正常妊娠组(NGT)和妊娠期糖尿病组,每组各40例,通过ELISA法检测母血血清、脐血血清Leptin水平,采用反转录-聚合酶链(RT-PCR)法和蛋白免疫印迹(Western blot)法检测胎盘组织中Leptin的mRNA及蛋白表达情况。结果母血血清、脐血血清中的Leptin水平在妊娠期糖尿病组孕妇中均高于正常妊娠组;胎盘组织中Leptin mRNA以及蛋白质的表达水平在妊娠期糖尿病组孕妇中均高于正常妊娠组,差异具有统计学意义(P<0.05)。正常妊娠组孕妇血清Leptin水平与脐血Leptin水平呈正相关(r=0.259,P=0.010)。妊娠期糖尿病组孕妇血清Leptin水平与脐血Leptin水平(r=0.248,P=0.012)、胎盘组织Leptin mRNA(r=0.826,P=0.007)、蛋白(r=0.628,P=0.015)及新生儿体重(r=0.358,P=0.015)、胰岛素抵抗指数(r=0.328,P=0.045)均呈正相关。结论 Leptin可能在妊娠期糖尿病的发生、发展中起着重要作用。
Objective To investigate the expression of Leptin in serum,cord blood and placenta of patients with gestational diabetes mellitus(GDM),and to explore the significance of Leptin in the occurrence and development of GDM.Methods Pregnant women who underwent cesarean section in our hospital from March,20 to March,2019 were selected for the study.A total of 80 selected pregnant women were divided into the normal pregnant women group(NGT)and GDM pregnant women group according to whether the mother had GDM,with 40 cases in each group.The levels of Leptin in serum and cord blood were detected by ELISA.The mRNA and protein level of Leptin in placenta was detected by reverse transcription-polymerase chain reaction(RT-PCR)and western blot.Results The Leptin levels in maternal serum and cord blood serum in the GDM group were higher than that in the NGT group(P<0.05).The expressions of Leptin mRNA and protein in placenta tissue of the GDM group were higher than that in the NGT group(P<0.05).Serum Leptin level was positively correlated with cord blood Leptin level in the NGT group(r=0.259,P=0.010).In the GDM group,serum Leptin level was positively correlated with umbilical cord blood Leptin level(r=0.248,P=0.012),placental tissue Leptin mRNA(r=0.826,P=0.007)and protein(r=0.628,P=0.015),as well as neonatal weight(r=0.358,P=0.015)and insulin resistance index(r=0.328,P=0.045).Conclusion Leptin might play an important role in the occurrence and development of gestational diabetes.
