摘要
目的探讨磁共振动脉质子自旋标记(ASL)成像在梗阻性肾病-肾纤维化模型中的应用。材料与方法将48只SD大鼠随机分为梗阻前组及梗阻后1、2、3、4、5 d组,每组8只。采用手术梗阻左侧肾脏,以右肾作为对照。使用3.0T磁共振进行肾脏ASL成像以获得双肾血流量(RBF)。扫描后处死动物并行病理学检查,将影像结果与病理结果进行对照。结果梗阻后1、2、3、4、5 d组大鼠肾脏RBF随梗阻时间延长而逐步下降,梗阻后5 d组大鼠梗阻侧RBF相当于对侧的29.3%。α-平滑肌肌动蛋白(α-SMA)阳性面积比逐渐升高,梗阻后5 d组大鼠梗阻侧α-SMA阳性面积比为(9.37±2.26)%,较对侧[(0.19±0.03)%]显著增高,差异有统计学意义(P<0.01)。梗阻后各组肾脏RBF与α-SMA阳性面积比呈负相关(r=-0.72,P<0.01)。结论磁共振ASL可作为一种无创且不使用造影剂的新技术用于检测梗阻性肾病-肾纤维化进程中RBF的变化,有助于反映肾脏疾病的早期病理生理改变。
Purpose To explore the application of arterial spin labeling(ASL)in obstructive nephropathy-renal fibrosis models.Materials and Methods Forty-eight SD rats were randomly divided into the pre-obstruction group and post-obstruction 1 d group,2 d group,3 d group,4 d group and 5 d group,with 8 rats in each group.The left kidney was obstructed surgically,and the right kidney was used as a control.3.0T MR was used to conduct renal ASL imaging to obtain the renal blood flow(RBF).After scanning,the rats were killed,pathological examination was performed,and the results of the MR were compared with the pathological findings.Results RBF of post-obstruction 1 d group,2 d group,3 d group,4 d group and 5 d group decreased gradually with the prolongation of obstruction time,obstruction-side RBF of the rats in post-obstruction 5 d group was equivalent to 29.3% of the opposite side.Theα-smooth muscle actin(α-SMA)positive area increased gradually,the obstruction-sideα-SMA positive area ratio of rats in post-obstruction 5 d group was(9.37±2.26)%,significantly higher than that of the opposite side(0.19±0.03)%,and the difference was statistically significant(P<0.01).The RBF was negatively correlated withα-SMA positive area ratio in post-obstruction groups(r=-0.72,P<0.01).Conclusion The magnetic resonance ASL may be used as a new kind of noninvasive technique to detect the changes in RBF in the obstructive nephropathyrenal fibrosis progress,and helps to reflect the early pathological and psychological changes in renal diseases.
