我院PIVAS针剂药品破损的原因分析及有效的防治措施 被引量:2

摘要 目的:分析我院静脉配置室2016年药品破损的原因,寻找合理有效的防治措施,以降低药品破损率,减少危险药品对PIVAS的污染,减少国家经济损失。方法:2016~2018年每月记录破损药品的药名、规格、数量和破损原因,分析原因,采取有效措施防治破损,对比干预前后的药品破损情况。结果:2016年针剂药品破损628个,主要原因是包装漏液、字迹模糊、物流运输过程破损、拆零过程摔破和加药过程中捏碎,一般原因是过期药品,次要原因是调剂差错和调剂过程摔破;通过有效的防治措施干预后,破损药品占药品总用量的比率有所下降;不同包装容器的药品破损占比都有减少;危险药品破损数量与占比率明显降低。结论:开展防治措施后,药品破损率有所控制,节约药品资源,减少危险药品对PIVAS的污染。 Objective:To analyze the causes of drug damage in our hospital’s pharmacy intravenous admixture service(PIVAS)in 2016,find reasonable and effective prevention measures to reduce the drug damage rate,to reduce the pollution of dangerous drugs in PIVAS and reduce the national economic losses.Methods:Record the name,specification,quantity and the cause of drug breakage from 2016 to 2018.Analyze the causes,take effective measures to prevent damage,and compare the drug breakage situation before and after the intervention.Results:628 injections were damaged in 2016.The main reason was that the bags were leaking,fuzzy writing,the logistics and transportation,the dismantling-breaking and the crushing in the dosing.The general reason was expired drugs.The secondary reason was that the adjustment error and the adjustment process broken.After the intervention of effective control measures,the ratio of damaged drugs to total drug consumption had decreased.The proportion of medicines in different packaging containers decreased,and the number of dangerous drugs breakage and proportion were significantly reduced.Conclusion:After carrying out prevention measures,the drug breakage rate was controlled,the drug resources were saved,and the pollution of the dangerous drug to the PIVAS was reduced.
