摘要
目的探讨COPD呼吸衰竭患者呼吸机相关性肺炎的有关因素,并就此给予相应的防护对策。方法收集2016年3月至2019年3月我院收治COPD呼吸衰竭患者86例,并就呼吸机相关性肺炎这项并发症进行研究,将患者分为发生组和未发生组,采用多因素logistic回归分析法对呼吸机治疗发生呼吸机相关性肺炎的主要危险因素进行分析。结果选取的84例患者中有9例出现呼吸机相关性肺炎,发生率为10. 47%。多因素logistic回归分析法显示肺部感染、机械通气时间≥7 d、留置胃管≥2 d为引发呼吸机相关性肺炎的独立危险因素(P <0. 05)。结论肺部感染、机械通气时间≥7 d、留置胃管≥2 d为导致COPD呼吸衰竭患者呼吸机相关性肺炎发生的主要因素,因此,明确上述因素,并给予相应的防护措施,为临床护理干预提供重要依据,减少呼吸机相关性肺炎的发生率,促进患者快速康复。
Objective To explore the relevant factors of ventilator-associated pneumonia in patients with chronic obstructive pulmonary diseases( COPD) respiratory failure,and to provide corresponding protective measures. Methods We selected 86 cases of patients with COPD respiratory failure who were admitted to our hospital from March 2016 to March 2019,and studied the complications of ventilator-associated pneumonia. The patients were divided into a occurrence group and a non-occurrence group. Logistic regression analysis was used to analyze the main risk factors of ventilator-associated pneumonia in ventilator therapy. Results There were 9 cases ventilator-associated pneumonia in the 84 cases of patients,and the incidence was 10. 47%. Multivariate logistic regression analysis showed that the pulmonary infection,mechanical ventilation time ≥ 7 d,and indwelling gastric tube ≥2 d were the independent risk factors to the occurrence of ventilator-associated pneumonia( P < 0. 05). Conclusion Pulmonary infection,mechanical ventilation time ≥ 7 d,and indwelling gastric tube ≥ 2 d were the main factors leading to the occurrence of ventilator-associated pneumonia in patients with COPD respiratory failure. Therefore,medical staff should clarify the above factors and give corresponding protective measures to provide important evidence for clinical nursing interventions,reduce the incidence of ventilator-associated pneumonia,and promote rapid recovery of patients.
