摘要
目的:探索上颌后牙游离缺失同时伴有种植位点牙槽骨高度不足4mm时,利用倾斜种植体植入上颌结节进行种植修复的效果。方法:选择上颌后牙游离缺失3个月以上的病例。在上颌结节倾斜植入种植体,3个月后冠修复,随访24个月以上。结果:共为41例患者上颌结节区倾斜植入42颗种植体,后期进行联冠或单冠修复。冠修复前总共3颗种植体出现脱落,修复前植体存留率92. 86%;修复2年存留率100%;1颗进行单冠修复的种植修复体第3年出现松动脱落。种植义齿功能良好。目前未脱落的种植体周围软组织健康、颈部边缘骨吸收速度<0. 2mm/年,修复体功能行驶良好。结论:当上颌后牙游离缺失伴余留牙槽骨高度不足时,在正确掌握适应症的前提下,配合规范的手术和修复操作,在上颌结节使用倾斜种植修复可收到满意效果。
Objective: To explore the effect of tilted implant in maxillary posterior teeth with free loss,When the alveolar bone height at the implant site was less than 4 mm. Methods: Cases of maxillary posterior free tooth loss for more than 3 months were selected. The implants were implanted obliquely in the maxillary tuberosity, and the crowns were restored after 3 months, with a follow-up of more than 24 months. Results: A total of 42 implants were implanted obliquely in the maxillary tuberous area in 41 patients, united crown or single crown was restored later. Before crown restoration, a total of 3 implants fell off, and the survival rate of implant before restoration was 92.86%;the 2-years survival rate after restoration was 100%;in the third year, a single crown implant became loose and detached. The function of implant denture were good.At present, the soft tissues of other implants were healthy, and the bone absorption rate at the neck edge was <0.2 mm/year.Conclusion: When maxillary posterior free tooth loss and the alveolar bone was not high enough, on the premise of proper indications and in combination with standard operations and crown repair, satisfactory results could be achieved in the restoration of maxillary nodules with inclined implantation.
