摘要
目的探讨扩散张量成像对脊髓炎型布氏杆病(MB)的诊断作用。方法搜集经血清学实验或病理确诊的21例脊髓炎型BS患者。采用3.0 T超导型磁共振扫描仪进行脊椎扫描,应用Fiber Trak软件包测量ADC值、FA值、γ1、γ2和γ3进行定量分析,分析ADC值、FA值与血清学凝集实验相关性,同时重建MB脊髓纤维束的改变形式。结果急性期、亚急性期A组、B组和C组的ADC值、FA值、γ1、γ2和γ3进行分析,差异有统计学意义(P<0.05),慢性期A组、B组和C组γ1值比较,差异无统计学意义(P=0.0.689),余参数比较差异有统计学意义(P<0.05)。后进行两两比较,急性期及亚急性A组、B组和C组的ADC值、FA值、γ1、γ2及γ3比较,其差异有统计学意义(P<0.05);慢性期A组、B组及C组的ADC值、FA值比较,其差异有统计学意义(P<0.05),γ1差异无统计学意义(P>0.05),B组与C组γ2、γ3比较γ1差异无统计学意义(P>0.05)。ADC值与血清凝集试验滴度呈正相关性,差异具有统计学意义(r=0.937,P=0.007);FA值与血清凝集试验滴度负相关性(r=-0.917,P=0.004)。通过ROC曲线分析显示,ADC值和γ1的AUC值敏感性、特异性及准确性高于相应FA值、γ2和γ3。MB的基本改变形式表现为:(1)彩码颜色改变;(2)缺失/断裂;(3)受压移位;(4)稀少。结论扩散张量成像不仅对MB具有诊断作用,还能分析ADC值、FA值与血清凝集试验滴度相关性,同时能直观发现脊髓纤维束各种改变形式。
Objective To investigate the diagnostic role of DTI in myelitis brachitis(MB). Methods 21 patients with MB were diagnosed by serological test or pathology from 2014.6 to 2017.7. Spinal scans were performed using a 3.0 T superconducting magnetic resonance scanner. The Fiber Trak software package was used to measure ADC values, FA values, γ1, γ2, and γ3 for quantitative analysis, and the correlation between ADC values, FA values, and serological agglutination experiments was analyzed. Reconstruction of altered forms of MB spinal fiber bundles was done. Results In the acute phase and subacute phase A group, the ADC value, FA value, γ1, γ2 and γ3 of group B and group C were analyzed. There was statistical difference(P<0.05) in chronic group A, group B and group C γ1. There was no statistical difference between the values(P=0.0.689), and the remaining parameters were statistically different(P<0.05). After the pairwise comparison, the ADC value, FA value, γ1, γ2 and γ3 of the acute phase and subacute group A, B group and C group were statistically different(P<0.05). In the chronic phase A and B group, there was a statistically significant difference between the ADC and FA values in the group and the C group(P<0.05). There was no significant difference in γ1(P>0.05). There was no significant difference in the γ1 between the B group and the C group γ2 and γ3(P >0.05). The ADC value was positively correlated with the serum agglutination test titer, with statistical difference(r=0.937, P=0.007). FA value was negatively correlated with serum agglutination test titer(r=-0.917, P=0.004). The ROC curve analysis showed that the sensitivity, specificity and accuracy of the ADC value and the AUC value of γ1 were higher than the corresponding FA values, γ2 and γ3. The basic change form of MB is as follows: 1 color code color change, 2 missing/breaking, 3 pressure shifting and 4 rare.
