摘要
目的探讨肝脏原发性少见肿瘤的MRI表现。结论回顾性分析经病理证实的15例肝脏原发性少见肿瘤的影像及临床资料,并结合相关文献进行总结。结果肉瘤样癌5例,MRI上T1WI呈低/稍低信号,T2WI呈高/稍高信号;增强扫描2例动脉期病灶边缘明显环形强化,延迟期呈廓清表现;3例动脉期病灶边缘轻度环形强化,延迟期持续强化;5例病灶内部实性成分呈结节状明显强化。间叶性错构瘤3例,MRI上T1WI呈低/稍低信号,T2WI呈等/稍高信号;1例为多房囊性结构,1例以实性成分为主,1例以囊实性成分为主;增强扫描1例为向中心填充性强化,2例动脉期囊壁、分隔及少许实性成分强化,延迟期持续强化。淋巴瘤3例,MRI上T1WI呈稍低信号,T2WI呈稍高信号,DWI呈明显高信号,增强扫描边缘呈轻度环形强化。另外神经内分泌癌2例,囊腺癌及平滑肌肉瘤各1例。结论肝脏原发性少见肿瘤的MRI表现有一定的特征性,在诊断和鉴别诊断中有一定价值。
Objective To study MRI findings of primary hepatic rare tumors. Methods Clinical information and MRI features of 15 histopathologically confirmed primary hepatic rare tumor patients were retrospectively reviewed, and combined with relevant literature to summarize. Results 5 cases of sarcomatoid hepatocellular carcinoma,MRI showed that the lesions were low/slightly lower signal on T1WI, high/slightly higher signal on T2WI;2 cases had obvious peripheral enhancement on the early phase,and clear performance on the delayed phase;3 cases had slightly peripheral enhancement on the early phase,and continuous enhancement on the delayed phase;The internal solid components of lesions showed obvious enhancement. 3 cases of Hepatic mesenchymal hamartoma,MRI showed that the lesions were low/slightly lower signal on T1WI,equal/slightly higher signal on T2WI;1 case was a multi-atrial cystic structure,1 case was mainly composed of solid components,and 1 case was mainly composed of solid capsules;1 case was Filling enhancement from focal edge to center,2 cases of arterial wall,separation and a little solid component were enhanced in the early phase and continuous enhancement displayed in the delayed phase. In 3 cases of primary hepatic lymphoma,MRI showed that the lesions were lower signal on T1WI,slightly higher signal on T2 WI and obvious high signal on DWI;3 cases showed slight marginal ring enhancement. In addition,there were 2 cases of primary hepatic neuroendocrine carcinoma and 1 case of cystadenocarcinoma and leiomyosarcoma. Conclusion Primary hepatic rare tumors have specific features on MRI which are valuable in diagnosis and differential diagnosis.
