Analysis of the status of multidrug-resistant organism infection in the hospital

摘要 目的分析医院临床多重耐药菌(MDROs)临床分布现状及耐药菌趋势变化情况,指导临床合理用药,为MDROs防控提供依据。方法回顾性分析医院2016年1月-2017年12月检出MDROs分布及体外药敏试验情况。结果医院共检出MDROs 1114株,MDROs感染发现率为1.69%,MDROs检出率为44.00%。MDROs检出按构成比分别为ESBLs大肠埃希菌22.26%、耐甲氧西林葡萄球菌17.77%、多重耐药肠杆菌14.90%、多重耐药铜绿假单胞菌14.45%、耐甲氧西林凝固酶阴性葡萄球菌11.22%、多重耐药鲍曼不动杆菌10.77%。临床分离MDROs前5科室为普外科、重症监护室、呼吸内科、儿科和神经外科。MDROs检出样本主要集中在痰样本、分泌物和中段尿样本。结论医院MDROs感染部位为下呼吸道、皮肤软组织和泌尿系统,应加强高危科室和易感染人群目标监测,监管抗菌药物合理使用,采取有针对性感染控制措施,遏制耐药菌播散。 Objective To analyze the clinical distribution and trend of MDROs in the hospital,to guide rational drug use and to provide evidence for the prevention and control of MDROs.Methods The distribution of MDROs and drug sensitivity in vitro were investigated retrospectively from January 2016 to December 2017.Results A total of 1114 MDROs strains were detected in this hospital.The detection rate of MDROs infection was 1.69%,and the detection rate of MDROs was 44.00%.MDROs were detected as ESBLs escherichia coli 22.26%,methicillin-resistant staphylococcus 17.77%,multi-drug-resistant enterobacter 14.90%,multi-drug-resistant pseudomonas aeruginosa 14.45%,methicillin-resistant coagulase-negative staphylococcus 11.22%,multi-drug-resistant acinetobacter baumannii 10.77%.The first five divisions of MDROs were general surgery,intensive care unit,respiratory medicine,pediatrics and neurosurgery.MDROs detection samples mainly concentrated in sputum samples,secretions and midstream urine samples.Conclusion MDROs infection sites in the hospital are lower respiratory tract,skin soft tissue and urinary system.Target surveillance should be strengthened in high-risk departments and susceptible populations,rational use of antibiotics should be monitored,and targeted infection control measures should be taken to curb the spread of drug-resistant bacteria.
