超声引导下颈2神经后支脉冲射频治疗颈源性头痛的疗效观察 被引量:6

Efficacy of Pulsed Radio Frequency of Cervical 2 Nerve Branches under Ultrasound Guidance in the Treatment of Cervical Headache

摘要 目的观察比较超声引导定位下颈2神经后支脉冲射频治疗与神经阻滞治疗颈源性头痛的安全性和有效性。方法选择颈源性头痛患者40例,分别接受超声引导定位进行C2神经后支脉冲射频治疗的20例为A组,接受超声引导下神经阻滞的20例为B组;记录治疗前后1、3、6月视觉模拟评分(Visual Analogue Scale,VAS),总有效率,不良反应发生率和复发率。结果治疗后A组各个时间点VAS评分较术前明显降低,优于B组,差异有统计学意义(P<0.05);术后6个月疗效显示,总有效率A组为95%,B组为85%,P<0.05;术后6个月复发率A组为5%,B组为15%。两组术中均无穿刺部位血肿、血管损伤、脊髓损伤、感染等并发症出现。结论超声引导颈2神经后支脉冲射频治疗颈源性头痛是一种安全、有效的微创治疗方法。 Objective To observe and compare the safety and effectiveness of radio frequency therapy with nerve block therapy in the treatment of cervical headache.Methods 40 patients with cervicogenic headache were selected.20 patients in group A were treated with radio frequency of C2 nerve posterior branch pulse under the guidance of ultrasound while 20 patients in group B were treated with nerve block,respectively.The visual analog scale(VAS)was recorded before and after the treatment in 1,3 and 6 months.The total effective rate,the incidence of adverse reactions and the recurrence rate were recorded.Results After the treatment,the VAS score of group A was significantly lower than that of group B(P<0.05).In addition,the total effective rate was 95%in group A and 85%in group B,respectively(P<0.05).Furthermore,the recurrence rate was 5%in group A and 15%in group B,respectively.There were no complications,such as hematoma,vascular injury,spinal cord injury,infection and so on.Conclusion It is a safe and effective minimally invasive method to treat cervicogenic headache by radiofrequency guided by ultrasound at the posterior branch of the cervical 2 nerve.
